G—csf支持下大剂量多疗程表阿霉素联合方案治疗实体瘤

摘　　要：目的 :探讨在一定剂量单核细胞集落刺激因子 (G—csf)支持下、大剂量多疗程表阿霉素联合方案治疗实体瘤的疗效及毒副反应。方法 :对同期住院诊断明确的 90例实体瘤患者 ,包括术前高危病例 ,随机分为A组、B组、C组。A组为G—csf支持下多疗程大剂量表阿霉素联合方案化疗组 ,B组为大剂量表阿霉素联合方案化疗组 ,C组为常规剂量化疗组 ,每组化疗均在 3个疗程以上。结果 :①A组化疗后WBC和ANC的最低值高于B组、C组 ,且WBC和ANC减少持续的日数少于B、C组 ,两者差异有显著性 (P <0 0 1) ;②大剂量表阿霉素联合化疗A、B两组CR +PR 36例 ,有效率 6 0 % ,中位缓解期 18 5 9个月 ,中位生存期 19 82个月 ,明显高于对照组C组 ,两者比较有显著差异 ,P <0 0 5。结论 :G—csf与大剂量表阿霉素在多疗程化疗中联合应用既可达到抗肿瘤提高疗效的目的 ,又可充分地动员外周造血干细胞 ,增加血象中性粒细胞及白细胞的数量避免由细胞毒药物所致的感染 ,从而保证了多疗程化疗的顺利进行 ,提高实体瘤的疗效及生存期 ,特别对乳癌、淋巴瘤、小细胞肺癌、生殖细胞瘤与化疗敏感性肿瘤适宜 ,并安全、费用低廉。Objective: To evaluate the efficacy and toxic side effect of massive dose and long-term application of Epirubicin supported by G-csf on treatment of solid carcinomas. Method: The 90 hospitalized patients of solid carcinoma were randomly divided in to 3 groups:group A was treated with massive dose Epirubicin supported by G-csf;group B with massive dose Epirubicn;group c with routine Epirubicin.The courses of treatment was more than 3 ones for every group. Results:① The minimum of WBC and ANC in group A post chemotherapy was significantly higher than that in group B and C ( P <0.01).The decreasing time of WBC and ANC in group A was significantly less than that in group B and C ( P <0.01). ②The CR and PR of group A and B was 36 cases,the effective rate was 60%,the disease median redian time(DRT)was 18.59 months and the median survival time (MST) was 19.82 months,which was remarkably higher than that in group C ( P <0.05). Conclusion: Combined applications of Epirubicin and G-csf can improve efficacy of anti-carcinoma,mobilize hemopoietic stem cell in peripheral blood,increase the number of neutrophillic granulocyte and leucocyte avoiding from infection of cytotoxic medicines and prolong survive time,specially for mammary cancer,lymphoma,small cell lung cancer,germinoma and sensitive cancer for chemotherapy.The treatment is safe and cheap.
