摘  要:目的 探讨周围型肺癌血流模式定量CT参数相互关系。方法  3 3例周围型肺癌 (直径≤ 4cm)患者 ,行多排螺旋CT(MSCT)动态增强 (以 4ml s的速度注入造影剂 90ml)。记录孤立肺结节增强前后各时相的CT值并计算强化值、灌注值 ,结节 -主动脉强化值比及平均通过时间。用线性回归分析评价主动脉强化值与孤立性肺结节的参数及各参数间的关系 ,并建立参数间的回归方程。结果 周围型肺癌强化值 (PHLC,3 7.61HU± 13 .17HU)与主动脉强化值呈正相关 (PHA,r =0 .558,Ρ =0 .0 0 1<0 .0 5)。肺癌与大动脉强化值比 (LC -to -Aratio,15.2 4± 4.47)及灌注值 (PLC,3 2 .12ml min 10 0g± 11.0 6)与动脉强化值无显著相关 (r=0 .10 3 ,Ρ= 0 .567>0 .0 5;r =0 .0 48,Ρ =0 .792 >0 .0 5)。肺癌灌注值与强化值及肺癌与大动脉强化值比呈正相关 (r =0 .40 9,Ρ =0 .0 18<0 .0 5;r =0 .456,Ρ =0 .0 0 8<0 .0 5)。肺癌强化值与肺癌与大动脉强化值比呈正相关 (r =0 .753 ,Ρ <0 .0 0 1)。PLC=14 .93 1+ 1.12 8×LC -to -Aratio。LC -to-Aratio=5.615+ 0 .2 56×PHLC。结论 可用二元一次方程式表示周围型肺癌灌注值、强化值及肺腺癌与大动脉强化值比的线性关系 ,设计简便的扫描程序研究周围型肺癌血管生成是可能的。Objective To evaluate the relationships am on g the quantifiable parameters derived with dynamic CT about blood flow pattern o f peripheral lung cancer (PLC). Methods 33 patients with peripheral lung cancer (diameter ≤4 cm)underwent multi-location dynamic contrast material-enhanced (90 ml,4 ml/ s) serial CT. Precontrast and postcontrast attenuation on every scan was recorde d. Perfusion ,peak height and ratio of peak height of the SPN to that of the aor ta were calculated. The relationships between peak height of the aorta (PH A)an d parameters of the PLC (PH LC , LC-to-A ratio and P LC ) and those amo ng parameters of the PLC were assessed by means of linear regression analysis. R egression equation among parameters of the PLC were obtain by means of stepwise regression. Results The correlation between the PLC peak height (PH LC ,37.61 HU ±13.17 HU)and the aortic peak height (PH A)was signifi cant(PH A ,r=0.558,Ρ=0.001<0.05). No significant correlatio n was found between the LC-to-A peak height ratio(15.24±4.47) and the aortic peak height(r=0.103,Ρ=0.567>0.05)as it was between the PLC p erfusion (P LC ,32.12 ml/min/100 g±11.06) and the aortic peak height (r=0.048,Ρ=0.792>0.05). The PLC perfusion correlated with th e PH LC and the LC-to-A peak height ratio(r=0.409,Ρ=0.018<0.05; r=0.456,Ρ=0.008<0.05). The PH LC correlated positively with the LC-to-A peak height ratio(r=0.753,Ρ<0.001). P LC =14.93 1+1.128×LC-to-A ratio. LC-to-A ratio=5.615+0.256×PH LC .Conclusion The linear relationship between the PLC perfus ion and LC-to-A ratio and that between LC-to-A ratio and PH LC can be expre ssed by equation. It is possible to design a simpler scanning procedure of inves tigation of peripheral lung cancer angiogenesis.
