螺旋CT双期扫描在胰腺肿瘤中的应用价值

摘　　要：目的评价螺旋CT双期增强扫描在胰腺肿瘤中的应用价值。方法收集41例胰腺肿瘤(36例胰腺癌、5例胰岛素瘤)病例,均作了螺旋CT平扫、动脉期、门脉期双期增强扫描。测定各期瘤灶与正常胰腺组织CT值,并计算两者差值。结果平扫、动脉期、门脉期的肿瘤灶与正常胰腺组织的CT差值分别为(6.7±4.3)HU,(31.6±14.3)HU,(4.9±3.8)HU。两者差值在动脉期与平扫、门脉期均有极显著统计学差异(t值分别为11.4,10.5,P均<0.001),平扫与门脉期无统计学差异(P>0.05)。结论螺旋CT动脉期、门脉期双期增强扫描在胰腺肿瘤诊断及鉴别诊断和小瘤灶的检出中具有重要价值。Objective To evalulate the clinical value of dual-phase spiral CT protocols in the diagnosis of pancreatic neoplasms.Methods 41 cases(36 with pancreatic carcinoma,5 with insulinoma)were performed with the protocols of spiral CT plain scan,arterial,and portal phase enhanced scan.Mean CT attenuation value was obtained for lesions and normal pancreatic parenchyma in the noncontrast and dual-phase images,respectively,and the δ value between the corresponding data was calculated.Results The δ value was (6.7±4.3) HU for plain scan,(31.6±14.7) HU for arterial and (4.9±3.8) HU for portal phase enhanced scan.There was apparently statistically significant between the arterial phase scan and the other two ones (both P< 0.001).The difference was not found statistical significance between the plain scan and portal phase (P >0.05).Conclusions The protocols of the arterial,portal dual-phase enhanced scan can provide an important value in the diagnosis and differeniation of pancreatic neoplasms and may be helpful for finding the small neoplasms.
