人工泪液对超声乳化术后泪膜的影响

摘　　要：目的 :观察白内障患者超声乳化手术前后泪膜的变化和应用人工泪液后对泪膜的影响。方法 :6 0例患者随机分为两组 ,A组超声乳化术后单用典必殊眼液滴眼 ,B组联合应用典必殊和泪然眼液滴眼 ,分别于手术前、手术后 1天、1周、1个月、3个月对 6 0例患者行棉线检查 ,角膜荧光素染色、泪膜破裂时间 (BUT)和SchirmerⅠ检查 ,对结果做统计学分析。结果 :(1)手术前A组和B组在棉线试验、角膜染色、BUT、SchirmerⅠ四个检查值经统计学分析无明显差异 ;(2 )两组术后 1天B组角膜染色分值小于A组 ,并且BUT较A组更长 ,统计学上有显著性差异 ,棉线试验和SchirmerⅠ试验检查B组虽有延长 ,但在统计学上没有显著性差异 ;(3)手术后 1周B组泪膜破裂时间比A组延长 ,差异有显著性 ,角膜染色、棉线试验和SchirmerⅠ试验检查两组在统计学上没有显著性差异 ;(4 )手术后 1个月B组在角膜染色评分小于A组 ,统计学上有显著差异 ,棉线试验、BUT和SchirmerⅠ两组无显著性差异 ;(5 )手术后 3个月B组角膜染色分值明显小于A组 ,泪膜破裂时间明显延长 ,统计学上有显著差异 ,棉线试验和SchirmerⅠ试验检查两组在统计学上没有显著性差异。结论 :白内障超声乳化术后早期泪膜稳定性明显下降 。Objective:To evaluate the tear film changes and the effects of artificial tears on the tear film after phacoemulsification.Methods:60 patients of cataract (60 eyes) were followed up for the tear film after phacoemulsification.Postoperatively,A group (30 eyes) were dropped with TobraDex,while B group (30 eyes) with TobraDex and Tear Naturale Ⅱ.Cotton thread test,corneal fluorescein staining,break-up time(BUT),Schirmer Ⅰ test(SⅠt) were observed preoperatively and 1 day,1 week,1 month,3 months postoperatively.The results were analysed statiscally.Results:(1)No significant differences existed in two groups before operation;(2) At 1 day postoperatively,significant differences existed in two groups,as corneal fluorescein staining were less and BUT were longer in group B;(3) At 1 week postoperatively,only BUT were longer in group B with significant difference;(4) At 1 month postoperatively,only corneal fluorescein staining were less in group B with significant difference;(5) At 3 month postoperatively,corneal fluorescein staining were less and BUT were longer in group B with significant diffenence.Conclusion:Tear film stability was decreased in earlier period after phacoemulsification,and artificial tears can accelerate the recovery of tear film.
