摘  要:目的:探讨微囊牛肾上腺嗜铬细胞(BCC)移植治疗晚期癌痛的有效性。方法:用胶原酶消化分离BCC,用海藻酸钠-聚赖氨酸-海藻酸钠(APA)包埋BCC,将APA微囊BCC植入急慢性疼痛动物模型大鼠和晚期癌痛患者的蛛网膜下隙观察镇痛效果。结果:APA微囊BCC对急慢性疼痛动物模型有明显镇痛作用。34例晚期癌痛患者的VAS评分移植前为8.86±0.94,移植后3天为2.36±1.81。移植后13例患者停用吗啡类药物(占38%),16例吗啡类药物用量减少(占47%),5例吗啡类药物用量无明显变化(占15%)。移植后生存3个月的16例患者中,8例吗啡类药物仍处于用量减少状态,8例无需应用。移植后1例患者生存5个月也未使用吗啡类药物。结论:APA微囊BCC对急慢性疼痛动物模型和晚期癌痛患者有明显的镇痛作用。Objective:To evaluate the effect of transplantation of microencapsulated bovine chromaffin cells(BCC)on pain stopping for the patients with advanced malignant tumors.Meth-ods:After BCCs were isolated withcollagenase and microencapsulated with Alginate-Polylysine-Al-ginate(APA),they were transplanted into the subarachnoid space of the pain models in rats and that of the patients suffering from intractable pain because of advanced malignancies.Then the changes of pain and quality of life were observed.Results:The microencapsulated BCCs played a significant analgesic role in the rats of pain model.The average VAS values of the34pa-tients were significantly lower after transplantation than before.The dosage of opiate was reduced in16(47%),and discontinued in13(38%)patients.Of 16patients who were still in life over3months after accepting the treatment,8did not need any and8needed less opiate continuously.Conclusions:The transplantation of microencapsulated BCCs with APA can play a significant analgesic role in the rats of pain model and the patients with advanced cancer pain.
