摘  要:目的 探讨肝细胞肝癌 (hepatocellularcarcinoma,HCC)中端粒酶、端粒酶催化亚单位hTERT(humantelomerasere versetranscriptase)表达及意义。方法 应用PCR ELISA和TRAP 银染法检测人HCC癌内、癌旁和正常肝组织中端粒酶活性 ,用RNA RNA原位杂交法检测石蜡包埋组织hTERTmRNA表达 ,观察二者与肿瘤病理特征间的关系。结果 癌组织中端粒酶活性、hTERTmRNA检出率分别为 81 3% (2 6 /32 )、87 5 % (2 8/32 ) ,远高于这些病例的癌旁肝组织 (1 2 5 % ,1 2 5 % ) ;癌内及癌旁端粒酶活性与hTERTmRNA表达相关。小肝癌和肿瘤分化差者端粒酶、hTERT均呈高表达 ;肿瘤近包膜处、与周边正常组织交错或移行处癌细胞hTERT表达强 ;癌内及癌旁浸润的单核性细胞、癌旁不典型增生区肝细胞及癌栓内的肿瘤细胞hTERTmRNA常为阳性。正常肝组织 4例均为阴性。结论 HCC中端粒酶活性、hTERTmRNA高表达 ,二者密切相关 ;端粒酶、hTERTmRNA活化表达与HCC的发生、演进及肿瘤的侵袭状态相关。癌旁端粒酶活性来自浸润的单核性细胞。Purpose To explore the significance of the telomerase activity and human telomerase reverse transcriptase (hTERT) expression in hepatocellular carcinoma (HCC). Methods Telomerase activity and hTERT mRNA expression in HCC and corresponding peri cancerous tissue as well as normal liver tissue were detected by PCR ELISA, TRAP Ag and in situ hybridization. The relationship between telomerase/hTERT expression and the pathologic features of tumor was also investigated. Results The positive rates of telomerase and hTERT mRNA expression in HCC were 81 3%(26/32) and 87 5%(28/32) respectively, higher than that in corresponding peri cancerous tissue (12 5% for both),while negative in normal liver. The expression of hTERT mRNA and telomerase activity showed a good correlation either in HCC or peri cancerous liver tissues. The telomerase/hTERT mRNA expression were positive in all small HCCs and poorly differentiated HCCs. Strong positive staining of hTERT mRNA was observed in the periphery region of tumor near its capsule or normal liver cells. The positive staining in peri cancerous tissue was found in the infiltrating mononuclear cells, dysplastic liver cells as well as cancerous emboli. Conclusions Higher telomerase activity and hTERT mRNA expression in most of HCCs suggest that the increase of telomerase activity/hTERT expression may be related to the pathogenesis of HCC as well as the extension and invasion of the tumor. The hTERT mRNA expression in mononuclear cells, dysplastic liver cells and tumor emboli may be the sourse of telomerase activity in non cancerous tissues.
