折刀位直肠癌根治术临床应用研究

摘　　要：目的　 探讨折刀位直肠癌切除保肛术的临床应用价值。方法　 对 2 6例低位直肠癌患者采用折刀位后路直视下肿瘤切除、腹膜外直肠吻合术。结果　 全组病例术后肛门排便功能满意 ,无手术死亡。术后切口感染 2例 ,手术并发症发生率为 7.6 9%。结论　 距肛缘 4～ 7cm的直肠肿瘤可以经骶尾后路在直视下从容切除并行腹膜外吻合 ,术式简捷、方便、安全 ,术后保留了肛门排便功能 ,提高了病人的生存质量。Objective To explore the clinical value of sphincter-saving resection of folding rectal carcinoma.Methods Twenty-six cases of low rectal carcinoma underwent carcinectomy through the posterior fold under direct vision and the extraperitoneal rectal anastomosis.Results The postoperative defecation function was satisfactory.No operative death occurred.2 cases suffered infection of incisional wound.The incidence rate of operative complication was 7.69%.Conclusion Rectal tumor about 4～7 cm from the perianal margin can be removed through the posterior sacrococcyx under direct vision and extraperitoneal rectal anastomosis can be performed.This kind of operation has proved to be simple and safe.The defecation function can be preserved postoperatively.The survival rates can be greatly improved.
