巨舌症的系列整复治疗1例

摘　　要：目的　总结巨舌症的治疗体会。方法　男性患者 ,3 2岁 ,舌肿块 3 6(口内 4cm ,口外 3 2cm× 15cm× 15cm) ,呈暗红色 ,表面多处散在溃烂 ;下唇肿块 10cm× 8cm× 5cm ,右颊、颌下、口底、颏下肿块 15cm× 10cm× 8cm ,质软。X线片示下颌骨前牙及牙槽突外翻畸形 ,牙弓扩大 ,后牙反牙合。治疗方案确定为分期手术治疗 ,即舌血管瘤切除、右颊、颌下、口底、颏下肿块切除、舌颌唇畸形整复术。结果　伤口均一期愈合 ,面部基本对称 ,舌运动存在 ,语音清晰 ,下颌骨运动正常 ,义齿修复后咬合关系恢复。结论　巨舌症可以采取分期手术治疗 。Objective To summarize the experience in the treatment of macroglossia. Methods A 32 year old man had a giant wine colored tumor, 36 cm×15 cm×15 cm, showing dispersed ulceration of the overlying mucosa. The patient was also presented with a mass of 10 cm×8 cm×5 cm in the lower lip and a soft mass of 15 cm×10 cm×8 cm extended to the right cheek, the submandibular space, the floor of mouth, and the submental space. X ray showed eversion of the lower incisor and cuspid and alveolar process, enlarged dental arch and posterior crossbite. The proposed treatment plan was surgical excision by stages, that is, lingual hemangioma excision followed by excision of the mass in the right cheek, the submandible, the floor of mouth, and the submental space, and then reconstruction of tongue, mandible and labial. Results All wounds were healed without infection. The patient recovered with symmetrical face, smooth tongue movement, clear postoperative speech, and functional occlusion. Conclusion Treatment of macroglossia by staging operation can result in satisfactory therapeutic effects.
