摘  要:目的 应用光学相干断层扫描 (opticalcoherenceto mography ,OCT)观测分析糖尿病性黄斑水肿患者行氩激光光凝视网膜水肿变化。方法  2型糖尿病患者黄斑视网膜水肿行氩 (绿 )激光光凝术 ,格栅状光凝 2 7眼 ,象限性光凝 14眼。光凝前 ,后 3、6个月及 6个月后分别重复同样扫描方式获取光学相干断层扫描图像。应用扫描仪自动测量程序记录黄斑水肿中心凹视网膜厚度 ,作统计学分析。结果 平均随访 13.1个月 ( 7~ 2 9个月 )。 6个月以后失随访 2眼。视力记录为 <0 .1,0 .1~ 0 .3,>0 .4 ;光凝前分别为 6、2 4、11眼 ;光凝后 3、6个月分别为 5、2 5、11眼及 3、2 4、14眼 ;随访终了时分别为 3、2 5、11眼。光凝前、后 3、6个月及 6个月后黄斑中心凹区视网膜厚度平均值分别为 378.2 ,30 1.0 ,2 10 .3和172 .1μm (我院正常值 14 0 μm± 12 μm)。统计学分析视网膜厚度光凝前与 6个月后 2组间差异有显著性意义 (t =2 .0 2 3,P <0 .0 5 ) ,余各组间差异无显著性意义。结论 光学相干断层扫描检测能较准确定量测算黄斑区视网膜厚度 ,可对比观察氩激光光凝前后视网膜厚度变化 ,进而判断糖尿病视网膜黄斑水肿变化。Objective To analyse the effect of argon laser photocoagulation in diabetic macular edema with optical coherence tomography.Methods Gridal or focal photocoagulation was treated for forty one eyes.The macular retinal thickness of every eye was examined with optical coherence tomography and computed automatically tomogram before treating and after treating 3,6 months,more than 6 months respectively.The average of macular retinal thickness was calculated from four quadrants in diffuse macular edema and calculated from affected quadrant in focal macular edema.Then these data were analyzed statistically.The visual acuity of every eye was recorded at the same time.Results The macular retinal thickness was 378.2μm before photocoagulation and was 301.0,210.3,172.1μm respectively after treating 3,6 months,and more than 6 months respectively(140μm±12μm in our hospital). It was significantly statistical difference between the macular retinal thickness before treating and after treating more than six months ( t=2.023,P <0.05).Conclusion Optical coherence tomography is able to quantify the development and resolution of diabetic macular edema after photocoagulation.It is a useful and non injury tool to analyse the effect of photocoagulation.
