人成釉细胞瘤中CD44_S及CD44_(V6)的表达

摘　　要：目的 :研究成釉细胞瘤 (ameloblastoma ,AB)和牙源性角化囊肿 (odontogenickeratocyst ,OKC)中CD44S 及CD44V6 的表达及其生物学的意义。方法 :采用免疫组化 (SP)法 ,分别对AB 77例 (原发 32例 ,复发 39例 ,恶变 6例 )CD44S、61例 (原发 2 4例 ,复发 31例 ,恶变 6例 )CD44V6 (因染色脱片故例数不一样 ) ,OKC19例 ,正常口腔粘膜 12例进行了研究。结果 :在正常口腔粘膜上皮棘深层、基底层细胞膜均有CD44S 和CD44V6 的完整表达 ,OKC中 10 /19例有CD44S 表达丢失 ,15 /19例有CD44V6 不完整表达或丢失。CD44S 在 2 1/77例AB中表达丢失 ,异常表达为 49/77例 ( 63.6% ) ;CD44V6 在 7/61例AB中为阴性 ,但异常表达却为 31/61( 5 0 .8% )。与正常口腔粘膜及OKC相比P <0 .0 0 1,伴随AB恶变阴性表达及异常表达率增加。结论 :在不同组织及病变中CD44S 和V6 有不同的表达 ,伴随AB复发与恶变Vs及V6 表达丢失和异常表达增加 。Objective: To study expression of CD44 S and CD44 V6 in ameloblastoma (AB) and odontogenic keratocyst (OKC) . Methods: S-P method was used to detect CD44 S protein in 77 cases of AB (32 of primary, 39 of recurrent,6 of malignant), 19 cases of OKC and 12 cases of oral normal mucosa;and CD44 V6 protein in 61 cases of AB (24 of primary , 31 of recurrent and 6 of malignant), 19 cases of OKC and 12 cases of oral normal mucosa. Results: Expression of CD44 S and CD44 V6 were detected in cell membrane of stratum spinosum and stratum basale in normal oral mucosa. In 19 cases of OKC, loss of expression of CD44 S was detected in 10 cases, and loss of integrity of CD44 V6 expression was detected in 15 cases. Loss ration of expression and abnormal expression ratio of CD44 S in AB were 27.3% and 63.6%, respectively. That of CD44 V6 in AB were 11.5% and 50.8% respectively. There was a signifcant difference between loss of expression or abnormal expression of CD44 S and CD44 V6 in AB, OKC and normal oral mucosa (P<0.01). Loss ratio of expression and abnormal expression ratio of CD44 S and CD44 V6 increased with AB transforming malignantly. Conclusion: CD44 S and CD44 V6 play an important role in genesis, loss of expression and abnormal expression are related with cell differentiation and proliferation of AB.
