深低温长期保存角膜穿透性移植的临床分析

摘　　要：目的 :分析深低温长期保存角膜穿透性移植术的临床效果和术后并发症。方法 :用深低温保存 1～ 60个月 (平均 5 5个月 )的角膜行部分穿透性移植术 2 5例 (2 5只眼 )。结果 :随访 1～ 3 6个月 (平均 15 6个月 ) ,18例植片透明 (72 0 % ) ,5例半透明 ,1例混浊 ,另 1例摘掉眼球。术后并发症有 :角膜水肿 (10 0 % )、虹膜粘连 (16 0 % )、并发性白内障 (16 0 % )、角膜裂开 (4 0 % )、继发性青光眼 (8 0 % )、排斥反应(2 4 0 % )、上皮持续缺损 (8 0 % )。结论 :深低温长期保存角膜的穿透性移植术后并发症部分可能与深低温保存有关 ,但仍是角膜移植的重要储备来源 ,具有重要的临床价值。Objective: To analyse the clinical effects and postoperative complications of penetrating keratoplasty with cryopreserved cornea. Methods: 25 cases (25 eyes) were performed penetrating keratoplasty with corneas cryopreserved for 1 to 60 months (averagely 5.5 months),and were followed up for 1 to 36 months (averagely 15.6 months). Results: After following up,18 cases of corneal grafts remained clear (72.0%),5 cases semitransparent,1 case cloudy and the other one had eyeball excavated.The postoperative complications included corneal edema (100%),iridocorneal adhesion (16.0%),complicated cataract (16.0%),corneal laceration (4.0%),secondary glaucoma (8.0%),graft rejection (24.0%) and persistent epithelial defect (8.0%). Conclusions: The cryopreserved cornea has clinical value for penetrating keratoplasty,but it may also be related to some postoperative complications.
