双栓塞治疗原发性肝癌合并脾功能亢进82例临床观察

摘　　要：探讨肝、脾动脉双栓塞治疗原发性肝癌合并脾功能亢进的疗效 ,对比观察治疗组 (肝、脾动脉双栓塞组 )与对照组 (单纯肝动脉化学栓塞组 )对血细胞、肝功能、免疫状况、门脉压力等方面的影响。治疗组 82例治疗后血细胞均见不同程度增高 ,血小板呈现倍增 ,PT、APTT同术前比有较明显改善 ;对照组 86例治疗后均有WBC、PLT的进一步减少 ,PT、APTT延长。两组肝肾功能变化无明显差异。免疫指标检测 :治疗组优于对照组 ,差异有显著性。治疗组门脉压力有较明显下降 ,未发生一例上消化道出血 (UGIB) ,对照组介入术后有 10例发生UGIB。肝脾动脉双栓塞治疗原发性肝癌合并脾功能亢进有较好疗效 ,能提高机体免疫功能 ,降低门脉压 。To study the efficacy on the treatment of hepatocellular carcinoma concomitant with hypersplenism using double embolization via hepatic and splenic arterialy. After a puncture via femoral artery, we catheterized into splenic arterialy and used gelfoam sponge as the embolic material for embolization, and then catheterized into hepatic arterialy for embolization, contrasted with the control group with a single TACE for the efficacy in principal aspects such as recovering liver and marrow reserve function, improving immunity of body, reducing tension of portal vein and so on. In test group which consists of 82 patients, blood corpuscles increased to some extent, PT and APTT improved notablely contrasted with the preoperative value, and blood platelet multipled from the former level after treatment; whereas in contrast group which consists of 86 patients PT and APTT prolonged, WBC and PLT decreased farther after treatment. There is no significant difference between two groups in liver function and kidney function, and the lesion of kidney function is not found. The laboratory examinations of immune function in the test group is remarkably greater than that of control group with significant difference. The tension of portal vein reduced obviously and UGIB has not taked place in test group, but there are 10 cases of UGIB in control group after TACE. The double embolization for the treatment of hepatocellular carcinoma concomitant with hypersplenism has presented curative effects on improving immunity of body and reducing tension of portal vein preferably, timesaving for whole treatment.
