原发性大肠淋巴瘤的CT检查价值

摘　　要：目的　探讨原发性大肠淋巴瘤的CT表现及其诊断价值。资料与方法　复习经病理证实的 8例原发性大肠淋巴瘤CT资料。所有病例均行常规腹部CT平扫 ,6例加作增强扫描 ,扫描层厚、层距为 5～ 10mm。结果　 8例原发性大肠淋巴瘤发病部位盲肠 3例 ,升结肠 2例 ,直肠 1例 ,大肠弥漫浸润 2例。CT表现主要包括 3种主要类型。局灶性肿块型 2例 ,其中 1例伴有肠套叠及腹膜后淋巴结肿大 ;节段环形浸润型 3例 ,其中 1例伴有肠系膜淋巴节肿大 ;弥漫浸润型 2例 ,呈多发节段性分布 ,累及整个结肠 ,并伴有肠系膜淋巴结肿大 ;直肠内的多发结节 1例。结论　CT能较准确地显示淋巴瘤的发病部位、并发症及与邻近结构的关系 ;局灶性肿块型、节段环形浸润型及弥漫浸润型是大肠淋巴瘤的 3种主要CT表现类型 ;正确辨认大肠淋巴瘤的CT表现 ,对部分病例可做出提示性诊断。Objective To study CT features of primary large intestine lymphoma and evaluate CT scanning in the diagnosis of the disease.Materials and Methods CT findings in 8 patients with pathologically proved primary large intestine lymphoma were retrospectively analyzed. Plain abdominal CT scanning was performed in all patients, additional enhanced CT scanning was carried out in 6 cases. Both the slice thickness and interval were 5～10 mm. Results The primary large intestine lymphoma was located at cecum (n=3), ascending colon (n=2), rectum (n=1) and diffusely infiltrating the whole colon (n=2). On CT scans, three types were mainly seen. Localized mass type was seen in 2 cases, of which intussusception and retroperitoneal adenopathy were accompanied in one. Segmental annular infiltrative type occurred in 3 cases, of which mesenteric adenopathy was demonstrated in one. Diffuse infiltrative type was seen in 3 cases, two of which showed multiple segmental lesions in the whole colon with mesenteric adenopathy and the other one showed multiple nodules in the rectum.Conclusion CT can accurately display the lesion's site and complications, as well as the involvement of the adjacent structures. Localized mass type, segmental annular infiltrative type and diffuse infiltrative type are three main patterns on CT scans. Suggestive diagnosis can be made in some patients when the CT features mentioned above can be recognized.
