胃癌的X线形态学与增殖细胞核抗原表达相关性研究

摘　　要：目的 :研究增殖细胞核抗原 (proliferatingcellnuclearantigen ,PCNA)与胃癌临床病理因素及X线钡剂造影表现的关系和意义。方法 :经低张气钡上消化道钡剂造影检查 ,确诊胃癌患者 35例 ,术后新鲜标本沿大弯 (或小弯 )剖开 ,摄取大体标本照片 ,再摄取标本涂钡照片 ;用免疫组化SP染色对 35例胃癌手术标本进行定量检测 ,观察PCNA与胃癌临床病理因素及X线钡剂造影表现的关系和意义。结果 :PCNA在胃癌组织中的阳性表达率明显高于癌旁及正常胃组织 (P <0 0 1) ,PCNA指数在高、中分化胃癌中的表达低于低分化胃癌 (P <0 .0 1) ,在淋巴结转移及浸润深度方面无统计学意义。观察胃癌标本涂钡照片 2 9例 ,分析癌肿周围粘膜破坏情况 ,在高、中分化胃癌中 ,癌肿组织与周围正常组织交界处粘膜皱襞中断截然的为 4 2 .86 % ,而在低分化胃癌中 ,80 %的癌肿与周围正常组织交界处粘膜皱襞中断截然 ,但经统计学处理P=0 .0 6 ,无统计学意义 ;在胃癌的大体分型、淋巴结转移及浸润深度方面 ,与粘膜分界也无统计学意义。结论 :检测胃癌组织中PCNA的表达有助于进一步了解肿瘤的生物学特性 ,PCNA可作为评价胃癌预后的指标 ;Objective:To investigate the correlation between PCNA and the X ray morphology,pathologic factors in gastric cancer.Methods:35 cases of gastric cancer which found with lower tonic air barium double contrast gastric examination were operated,and the specimens were photographed by Olympus digital camera and then recorded with X ray film after evenly coated with barium in the surface of membrane .The expression of PCNA was studied on 35 cases of gastric cancer by SP immunohistochemical technique.X ray morphology and pathologic factors were examined.Results:There was a significant difference in PCNA expression between in the gastric cancer and in the paracancerous tissues and normal tissues(P<0.01),the PCNA index in poorly differentiated type was higher than in well/moderately differentiated type. No significant correlation was found between expression of PCNA and lymph node metastasis,grade of differentiation. 29 cases were analyses to indicate the abruption of mucosal folds,there were complete division between the carcinoma and the normal tissues in poorly differentiated type,in well/moderately differentiated type,there were obscure division,but they were no statistics correlation. No significant correlations were found between abruption of mucosal folds and lymphonodus metastasis,grade of differentiation.Conclusion:It is helpful for predicting biological characteristics of gastric carcinoma to examine the expression of PCNA;No significant correlation was found between X ray morphology in gastric cancer and the expression of PCNA.
