下睑肿瘤切除术后耳软骨移植Ⅰ期下睑再造

摘　　要：目的 :探讨下睑肿瘤切除术后用自体耳软骨移植行下睑Ⅰ期再造修复的效果。方法 :2 1例下睑肿瘤患者于肿瘤切除后充分分离残余的皮肤、眼轮匝肌、睑结膜至穹窿 ,固定耳软骨将下睑皮肤、轮匝肌与结膜对位缝合 ,于下睑穹窿部做褥式缝合 ,牵引固定于上睑皮肤处。结果 :2 1例中随访 15例 ,随访 2～ 5年。 15例保持了下睑的正常功能 ,眼睑活动自如 ,闭合完全 ,无内翻倒睫。结论 :用自体耳软骨行Ⅰ期下睑再造修复 ,既减少患者多次手术痛苦 ,减轻了经济负担 ,又无排斥反应 ,取得了满意效果。Objective:To evaluate the effect of the first-phase lower lid reconstitution with ear cartilage graft after excision of tumor.Methods:In 21 patients with tumors of lower lid,after the tumors were excised,the remnant skin,musculi orbcularis oculi and palpebral conjunctiva were dissected to the fornix and ear cartilage graft was rivelted,then sutured the lower lid skin,musculi orbicularis and conjunctiva contrapositionly.Make a mattress suture at lower fornix,passing through the skin and fixed on the upper lid skin.The sutures were removed 7～10 days postoperatively.Results:In 15 of 21patients at the 2～5 years follow-up,the lower lid kept good looks and normal functions,without inversion or trichiasis.Conclusion:This operation of the first-phase lower lid reconstitution with ones own ear cartilage reduces the pain of the patient caused by repeated operations and economic burden.It has no rejection and gives a satisfactory result.It is recommended in the operations for lower lid tumors.
