食管癌机械吻合及带蒂胸膜片包埋吻合口方法研究

摘　　要：目的探讨食管癌行机械吻合及应用带蒂胸膜片包埋吻合口的优点。方法对食管中下段癌采用机械吻合器行食管、胃主动脉弓上吻合或胸顶吻合,并以带蒂胸膜片包埋吻合口;同期行常规手工吻合及胃壁包埋吻合口作为对照组。结果机械吻合组术后吻合口瘘发生0.6%,死亡1例,吻合口狭窄发生7%,无死亡,手术死亡率0.2%;对照组吻合口瘘发生2.3%,死亡5例,吻合口狭窄发生2.7%,无死亡,手术死亡率1.9%。结论食管胃机械吻合及应用带蒂胸膜片包埋吻合口的方法术后吻合口瘘发生率低,死亡率低,但吻合口狭窄发生率高。objective: To investigate the approach of wrapping the stoma with pediceld pleura chaff after stapled anastomosis in esophagogastrectomy for esophageal carcinoma. Methods: We adoptted the approach of wrapping the Stoma with pediceld pleura chaff after stapled anastomosis in esophagogastrectomy for patients with esophageal carcinoma, and patients were treated in routine technique at the same time, and evaluated the therapeutics effect. Results: Anastomotic leakage rate was 0.6%, 1 case dead the anastomotic constriction was 7%, the death rate of perioperation was 0.2%. In the control group, the anastomotic leakage rate was 2.3%, 5cases dead the anastomotic constriction rate is 2.7%, the death rate was 1.9% respectively. Conclusion: The approach of wrapping the stoma with pediceld pleura chaff after stapled anastomosis in esophagogastrectomy for esophageal carcinoma is effective with low anastomotic leakage rate, but with higher anastomotic constriction occurrence.
