摘 要:目的探讨caspase - 3,bcl- 2与卵巢上皮性肿瘤发生、发展的关系。方法利用免疫组化(SP)法和TdT介导的dUTP的缺口末端标记 (TUNEL)技术 ,检测 72例卵巢上性肿瘤组织中caspase - 3和bcl- 2蛋白、PCNA的表达水平及细胞凋亡情况。结果caspase - 3在卵巢恶性肿瘤中的表达 (44 .2 % ) ,明显低于良性肿瘤 (83.3% ,P <0 .0 5 ) ;在低分化卵巢癌中的表达 (76 .7% ) ,明显高于高分化卵巢癌 (47.1% ,P <0 .0 5 )。bcl- 2蛋白在卵巢恶性肿瘤中的表达 (76 .6 % ) ,明显高于良性肿瘤 (2 5 .0 % ,P <0 .0 1) ;在低分化卵巢癌中的表达 (42 .1% ) ,明显低于高分化卵巢癌 (90 .0 % ,P <0 .0 1)。凋亡指数与增殖指数呈正相关 (r =0 .4 82 ,P <0 .0 1) ,与caspase - 3的表达呈正相关 (r =0 .6 4 7,P <0 .0 1) ,与bcl - 2的表达呈负相关 (r =- 0 .5 31,P <0 .0 1)。结论caspase - 3,bcl- 2与卵巢上皮性肿瘤的细胞凋亡密切相关 ,参与细胞凋亡的调节机制 ,在卵巢上皮性肿瘤恶变的过程中发挥作用 ,并与恶性程度关系密切 ,有可能成为卵巢癌预后评估的参考。Objective: To explore the relation of caspase-3 and bcl-2 protein with the occurrence and development of epithelial ovarian tumor. Methods: Seventy-two cases of primary ovarian tumor were studied by using immunohistochemistry and TdT-mediated dUTP nick end labeling (TUNEL) method, to examine the expression of caspase-3, bcl-2, proliferating cell antigen (PCNA) and the condition of cell apoptosis. Results: The positive rate of caspase-3 in malignant ovarian tumor was 44.2%, which was obviously lower than the expression in benign tumor (83.3%, P<0.05). The expression of caspase-3 in malignant ovarian tumor was correlated with its differentiation and stage (P<0.01). The positive rate of bcl-2 in malignant ovarian tumor was significantly higher than in benign tumor (P<0.01). while decreased with the decreasing of malignant ovarian tumor's differentiation (P<0.01). Apoptosis index correlated positively with proliferating index (r=0.482, P<0.01) and the expression of caspase-3 (r=0.647, P<0.01), while inversely with the expression of bcl-2 (r=-0.531, P<0.01). Conclusion: There is close relation between the expression of caspase-3, bcl-2protein and cell apoptosis in epithelial ovarian tumor. caspase-3, bcl-2 may participate in the regulating mechanism of cell apoptosis and have function in changing malignance. The expression of caspase-3 and bcl-2 protein have close relation with malignant degree of ovarian cancer and might be reference of estimating the prognosis.
