急性脑梗死血清肿瘤坏死因子-α、白介素6水平与脑底动脉血流变化的关系

摘　　要：目的 探讨急性脑梗死患者血清肿瘤坏死因子-α(TNF-α)、白介素6(IL-6)与脑底动脉血流变化关系。方法对112例急性脑梗死患者进行血清TNF-α及IL-6测定,并通过经颅多普勒超声(TCD)检测分成脑动脉狭窄组及脑动脉供血不足组,计算脑血管搏动指数(PI)及阻力系数(RI),并与40例正常人做对照。结果 脑动脉狭窄组的PI、RI、血清耵TNF-α及IL-6的含量明显高于对照组(P<0.01),较脑动脉供血不足组显著增高(P<0.05)。脑动脉供血不足组血清TNF-α及IL-6含量较对照组显著增高(P<0.05)。结论 急性脑梗死患者血清TNF-α、IL-6与脑底动脉血流变化密切相关。脑底动脉狭窄者较脑底动脉供血不足者脑组织缺血坏死程度及炎性反应更明显,更应引起临床高度重视。Objective To investigate the relationship between changes of serum tumor necrosis factor-α(TNF-α), interleukin-6 (IL-6) and basal arterial blood flow in patients with acute cerebral infarction.Method 112 patients with acute cerebral infarction were examined for serum TNF-α,IL-6 and transcranial Doppler (TCD). Cerebral vascular pulsative index (PI) and resistance index (RI) were calculated.The patients were divided into two groups: one group with narrowing of cerebral artery and the other with blood insufEciency in cerebral artery by TCD and were compared with 40 normal controls. Results PI, RI, serum TNF-α and IL-6 levels in group with narrowing of cerebral artery were higher than those in normal control group ( P < 0.01) and group with blood insufficiency(in cerebral artery, P < 0.05) . The serum TNF-a and IL-6 levels in group with blood insufficiency in cerebral artery were significantly higher than these in control group( P <0.05).Conclu-tion There is close relationship between serum TNF-α, IL-6 and basal arterial blood flow in patients with acute cerebral infarction.The degrees of ischemia, necrosis and inflammation in patients with narrowing of cerebral artery are more obvious than those in patients with blood insufficiency in cerebral artery, which should call the attention of clinical doctor.
