眶—球硅连术治疗大幅度麻痹性斜视

摘　　要：目的:为探索用硅胶管作眶—球连接术治疗麻痹性斜视的疗效。方法:对已超过40°的10例(10眼)麻痹性内、外斜视的患者进行了手术,当完成拮抗肌的后退后,在外眦部眶骨膜与麻痹肌止端前巩膜或止端后巩膜进行硅胶管连接。结果:术后随访6～12月,8例眼位矫正至正视位;欠矫和过矫10°各1例。因硅胶富有弹性,所有术眼还可向麻痹肌侧转动10%～75%的幅度。结论:因在眼眶骨膜和眼球间所连接的硅胶起着一种被动牵拉作用,故该技术是矫正麻痹性斜视中可取的较好的方法之一。AIM: To discuss the effect of orbit-orb connection using silicone tube in treating large angle paralytic strabismus.· METHODS: Operations were performed on 10 cases (10 eyes) of esotropia or extropia. After antagonist retrocession, the anterior or posterior sclera of paralitic rectus insertion was connected to periorbita near the lateral or medial canthus by using a small silicone tube. · RESULTS: During the follow-up of 6-12 months, the orthophoric effect was satisfactory in 8 cases, and undercorrection and overcorrection of 10° squint occurred in 1 case respectively. Owing to the elasticity of silicone, all the operated eyeballs could voluntarily abduct or adduct 10%-15% of amplitude in ocular vnobility toward paralitic side. · CONCLUSION: The silicone connecting periobita and eye ball plays passive tractive role, thus orbit-orb silicone connection is a proper method for the commection of paralytic strabismus.·
