摘 要:目的 :探讨脑梗死患者急性期白细胞CD1 1a,CD1 8和LFA 1的表达及其免疫阳性细胞数与梗死灶体积的关系 .方法 :流式细胞技术测定 30例脑梗死患者急性期 (发病 72h内 )和 2 8例健康人多型核细胞 (PMN)、单核细胞 (MN)和淋巴细胞 (LC)CD1 1a,CD1 8和CD1 1a /CD1 8(LFA 1 )的表达 ,用阳性细胞百分率表示 (% ) ,并计算外周血中PMN ,MN和LCCD1 1a ,CD1 8,LFA 1免疫阳性细胞数 .结果 :脑梗死患者急性期PMN ,MN和LCCD1 1a ,CD1 8,LFA 1表达阳性百分率明显高于健康对照组 (P <0 .0 5 ) ,在大 (大于 1 0cm3 )、中 (5cm3 ~ 1 0cm3 )、小 (小于 5cm3 ) 3型不同梗死灶体积的脑梗死患者中 ,各型之间表达细胞百分率均无显著性差异 (P >0 .0 5 ) .外周血PMN表达CD1 1a,CD1 8和LFA 1的阳性细胞数在 3型梗死灶之间及与正常对照组之间均存在显著性差异 (P <0 .0 5 ) ,表达阳性细胞数与梗死灶体积的大小有显著相关 (r1 =0 .396 ,P <0 .0 5 ;r2 =0 .987,P <0 .0 1 ;r3 =0 .794 ,P <0 .0 1 ) .MN和LCCD1 1a,CD1 8,LFA 1表达阳性细胞数在 3型梗死灶的患者中均无显著性差异 (P >0 .0 5 ) ,而与正常对照组之间均有显著性差异 (P <0 .0 5 ) .结论 :脑梗死急性期PMN ,MN和LCCD1 1a ,CD1 8,LFA 1表达上调 ,外周血中PMNCD1 1a,CD1 8?AIM: To study the relation between different infarct sizes and the expressions of CD11a, CD18, LFA 1 as well as their positive cell numbers on leukocytes of peripheral blood in acute phase patients with cerebral infarction. METHODS: The expressions of CD11a, CD18 and LFA 1 on the polymorphonuclears (PMN), the mononuclears (MN) and the lymphocytes (LC) in peripheral blood were measured, by immunofluorescence phenotype and flow cytometry, in 30 patients with cerebral infarction within 72h after onset and 28 control subjects. The total positive cells (×10 9/L) of the expressions of CD11a, CD18 and LFA 1 on the leukocytes in peripheral blood were calculated and comparisons were made between the patients and the controls. RESULTS: The expressions of CD11a, CD18 and LFA 1 on the PMN, the MN and the LC in patients were higher than those in controls ( P <0.05), but no significant difference was found between those with infarct sizes >10 cm 3, the 5 cm 3 -10 cm 3 and the <5 cm 3 ( P >0.05). Great difference of CD11a, CD18 and LFA 1 expression on PMN was observed in patients with different infarct sizes, and between patients and the control ( P <0.05). The number of positive cells was positively correlated to the infarct size ( r 1 = 0.396, P <0.05; r 2 = 0.987, P < 0.01; r 3 = 0.794, P < 0.01 ). No significant difference of CD11a, CD18 and LFA 1 expression on MN and LC was found in patients with different infarct sizes ( P >0.05), while significant difference was found between patients and the control ( P <0.05). CONCLUSION: The expressions of CD11a, CD18 and LFA 1 on the PMN, the MN and the LC in peripheral blood increases in acute phase patients with cerebral infarction, and the increased expression of CD11a, CD18 and LFA 1 on PMN is correlated to the infarct size.
