藏族和汉族胃癌相关抗原MG_7-Ag表达差异的分析

摘　　要：目的　研究藏、汉族胃癌相关抗原MG7 Ag表达状况。方法　采用ABC免疫组化方法检测 2 0 0例胃癌手术标本 (藏、汉族各 10 0例 )中MG7 Ag的表达 ,同时从两民族随机分别选取 18例胃黏膜不典型增生 ,10例慢性萎缩性胃炎 ,10例正常胃黏膜作为对照研究。结果　MG7 Ag阳性检出率在汉族、藏族胃癌中分别为 88 7%、92 4 % ,在慢性萎缩性胃炎中分别为 10 0 %、2 0 0 % ,在胃黏膜不典型增生中分别为 38 9%、5 5 6 % ,正常胃黏膜MG7 Ag表达均为阴性。从正常胃黏膜、慢性萎缩性胃炎、胃黏膜不典型增生到胃癌 ,MG7 Ag阳性率呈递增趋势 ,在同一民族中与正常胃黏膜相比 ,慢性萎缩性胃炎、胃黏膜不典型增生及胃癌中MG7 Ag的表达率均显著升高 (P <0 0 1) ,但两民族间差异无显著性 (P >0 0 5 )。在两民族间MG7 Ag的阳性表达与胃癌有无淋巴结转移及胃癌的分化程度无相关性 (P >0 0 5 ) ,而在同一民族间胃癌的MG7 Ag阳性表达与胃癌有无淋巴结转移及胃癌的分化程度有相关性 (P <0 0 1) ,其差异主要表现在中分化腺癌与低分化腺癌之间。结论　MG7 Ag在两民族胃癌中的表达状况差异无显著性 ,因此在藏族胃癌中MG7 Ag可作为可靠的肿瘤标记物。Objective To investigate the difference of the expression pattern of tumor-associated antigen MG_7-Ag in gastric carcinoma between Hans and Tibetans. Methods Avidin-biotin peroxidase complex immunohistochemical methods were adapted to examine the expression pattern of MG_7-Ag in 200 patients with gastric carcinoma ,including 100 Tibetan patients and 100 Hans patients ,10 patients with chronic artrophic gastritis ,18 with gastric mucosal dysphasia and 10 subjects with normal gastric mucosa served as control. Results The positive rate for MG_7-Ag was 92.4% and 88.7% in Tibetan and Han gastric carcinoma patients,55.6% and 38.9% in Tibetan and Han gastric mucosal dysplasia patients,20.0% and 10.0% in Tibetan and Han chronic atrophic gastritis patients,respectively. There was no positive expression in normal gastric mucosa of both Hans and Tibetans. From normal gastric mucosa,chronic artrophic gastritis and gastric mucosal dysplasia to gastric carcinoma ,expression of MG_7-Ag showed an ascending tendency in the same group of subjects or patients ( P <0.01). However between Hans and Tibetans there was no significantly different expression of MG_7-Ag. In gastric carcinoma ,the expression pattern of MG_7-Ag was not related to tumor cell differentiation and metastasis of lymph nodes in Hans and Tibetans ( P >0.05),but in the same group the expression of MG_7-Ag was related to metastasis of lymph nodes. Conclusions The expression pattern of MG_7-Ag displayed no significant difference between Hans and Tibetans .The tumor associated antigen MG_7-Ag in gastric carcinoma can be used in Tibetans as a reliable marker to predict early gastric cancer.
