盐酸氟西汀联合使用激素治疗更年期抑郁症对照研究

摘　　要：目的 :探讨女性更年期抑郁症的治疗效果 ,寻求合理的治疗方法。 方法 :对 46例女性更年期患者随机分为两个治疗组 ,其中单用一般三环抗抑郁剂 2 2人 ,抗抑郁剂 +激素替代治疗 2 4疗例 ,对治疗前和治疗后分别HAND、HAMD评分。以治疗前后HAND、HAMD的减分统计疗效。 结果 :单用三环抗抑郁剂治疗组有效率为 72 .7% ,治愈率为 2 7.3 % ;盐酸氟西汀 +激素替代治疗组有效率为 95 .8% ,治愈率为5 4% ,两个治疗组之间抗抑郁疗效差异有显著性 (P <0 .0 5 ) ,盐酸氟西汀 +激素替代治疗组疗效明显优于单用三环抗抑郁剂治疗组(P <0 .0 5 )。结论 :盐酸氟西汀 +激素替代治疗对女性更年期抑郁症疗效较好 ,尤其对于中、重度抑郁者 ,是值得推荐的治疗方法。Objective: To explore the curative effect of woman menopause depression and look for the best treatment. Methods: Divided 46 women patients with menopause depression into two groups: 24 cases of the experiment group were cured with fluoxetine and hormone substitute cure, and 22 cases of control group were cured with ordinary antidepressant (amitriptyline). All the patients were assessed with HAMD and HAMA before and after treatment. The curative effect was evaluated according to the difference of HAMD or HAMA score before and after treatment. Results: In control group, the effective rate was 72.7% and curative rate was 27.3%. In the experiment group, the effective rate was 95.8% and curative rate was 54%. The difference between the two groups was significant (P<0.05). The curative effect of fluoxetine combination with hormone substitute cure was better than amitriptyline (P<0.05). Conclusions: Fluoxetine combination with hormone is better to cure woman menopause depression, especially for the severe patients.
