环孢霉素A在兔青光眼滤过手术中抗瘢痕形成的实验研究

摘  要:目的评估兔青光眼滤过性手术中局部应用环孢霉素A(Ciclosporin A,CsA)后抑制滤过瘢痕形成的效果及与保护视功能的关系。方法随机选择新西兰大白兔20只40眼,右眼20眼为CsA组,左眼20眼为丝裂霉素C(Mitomycin C,MMC)组。分别在术中一次性应用2%CsA、0.2mg/ml MMC 5 min,术后定期观察眼部情况,滤过泡及眼压等,并于术后第2个月、第3个月时,取滤过口附近组织送病理、电镜检查。结果 (1)CsA组中功能性滤过泡的眼占90%,MMC组占75%(P<0.05);(2)术后24h波动眼压峰值CsA组为2.34±0.35 kPa,MMC组为2.53±0.72 kPa(P<0.05)。结论 2% CsA可有效地减少青光眼术后滤过道瘢痕形成,提高小梁切除术的成功率。Purpose To evaluate the effect of CsA on anti-fibfroblast proliferation on operative site after the trabeculectomy in glaucoma in New Zealand rabbits. Methods 20 cases (40 eyes) of New Zealand rabbits are randomly studied by treatment with 2% CsA or 0.2 mg/ml MMC for 5 minutes,for each group. Slit-lamp examination and tonometry were done after operation histopathological changes were studied. 2 months and 3 months postoperatively.Results (1)There were 90% functional filtering blebs in the CsA group. and 75% in the MMC group (P< 0.05) . (2) The postoperative mean diurnal peak IOP were 2.34±0.35 kPa (CsA group) ,2.53 ±0.72 kPa (MMC group) respectively (P<0.05) .Conclusion 2% CsA can effectively inhibit the scar formation and lower down the IOP to a desired degree, thus providing a higher successful rate of trabeculectomy.
