摘 要:目的 探讨细胞凋亡和细胞周期相关基因蛋白与经典型霍奇金淋巴瘤临床分期的关系。方法 应用免疫组化ABC法检测bcl 2 ,ABC和TUNEL双标记技术检测细胞凋亡 ,ABC和SABAP双标记方法检测MIB1、TopoⅡα、Rb和 p2 1在 6 2例经典型霍奇金淋巴瘤组织芯片中的表达。结果 6 2例经典型霍奇金淋巴瘤中 35例bcl 2阳性 ,在 6 2例非肿瘤性背景细胞中均有凋亡小体 ,而在H RS细胞仅有 10例胞质内见凋亡小体 ;H RS细胞bcl 2表达与细胞凋亡呈负相关性 (P <0 0 5 ) ;MIB1及TopoⅡα高表达与临床分期呈正相关性 (P <0 0 1) ,反之 ,Rb及p2 1高表达与临床分期呈负相关性 (P <0 0 1)。所有抗原的表达与霍奇金淋巴瘤的组织学亚型均无相关性 (P >0 0 5 )。结论 bcl 2的过度表达可以阻断H RS细胞的凋亡 ;CD30阳性H RS细胞中MIB1及TopoⅡα表达指数随病程的进展而升高 ,而Rb及 p2 1表达指数随病程的进展而降低 ,他们可作为霍奇金淋巴瘤预后判断的参考指标。Purpose To investigate the protein expression patterns of apoptosis and key cell cycle-related genes in relation to clinical stages in classicle Hodgkin's lymphomas (CHL). Methods An ABC method was used to detect the expression of bcl-2, a double immunohistochemical and TdT-mediated dUTP-FITC nick end-labelling(TUNEL)technique was used to detect the apoptosis and,a double ABC-SABAP method was used to detect the expression of MIB1, TopoⅡα, Rb and p21 on tissue microarray sections of 62 cases of CHL. Results The H-RS cells of 62 CHL were bcl-2-positive in 35 cases. Apoptosis was found in the non-neoplastic cellular background in all cases but in H-RS only 10 in 62 cases. There was a reverse significant correlation between expression of bcl-2 and apoptosis (P<0.05). A positive relation was found between expression of MIB1, TopoⅡα and stages (P<0.01). By contrary, a reverse relation was found between expression of Rb, p21 and stages(P<0.01). No correlation was found between expression of any of these antigen markers and the sub-types in CHL (P>0.05).Conclusions Over expression of bcl-2 may be linked to blockage of apoptosis of H-RS cells in CHL. The index of MIB1 and TopoII expression in CD30 positive H-RS cells increases with the advanced stages. However, the index of Rb and p21 expression in CD30 positive H-RS cells is consistent reversely with the stages. The MIB1,TopoⅡα, Rb and p21 expression in CD30 positive H-RS cells can be used as a reference for the prognosis in CHL. The progression of CHL depends on protein expression patterns of apoptosis and key cell cycle-related genes but not sub-types.
