摘 要:目的探讨α1 -抗糜蛋白酶 (α1-antichymotrypsin,α1-ACT )杂合缺失是否为广州市儿童哮喘发病的遗传机制以及地理环境是否影响α1-ACT杂合缺失在儿童哮喘中的分布。方法应用火箭免疫电泳技术对广州市50例哮喘儿童、50例健康儿童及100例健康成人血浆α1 -ACT含量进行检测 ;经家系调查确定3组α1-ACT杂合缺失频率 ;比较了哮喘组α1-ACT杂合缺失及非缺失患儿的临床资料。结果α1-ACT杂合缺失频率哮喘组、儿童对照组以及成人对照组依次为14 %、2 %、1 % ,哮喘组较儿童对照组明显增高 (χ2=4.891,P<0.05) ;广州市哮喘儿童α1-ACT杂合缺失频率 (14 % )明显高于重庆市哮喘儿童 (4.4 % ) ,χ2 =4.054,P<0.05 ;哮喘组α1 -ACT杂合缺失与非缺失患儿比较 ,前者哮喘住院次数多 ,放射性过敏原吸附试验阳性率显著增高。结论α1-ACT杂合缺失与儿童哮喘发病存在一定的联系 ;地理环境影响α1 -ACT杂合缺失频率分布。Objective To determine the association of heterozygous deficiency ofα 1 -antichymotryp- sin(α 1 -ACT)with asthma and the geographical distribution of childhood asthma in Guangzhou.Methods The plasma levels ofα 1 -ACT were determined in50children with asthma,50healthy children,and100healthy adults with rocket immunoelectrophoresis.Three types of heterozygous deficiency ofα 1 -ACT were observed with pedigree investigation.The clinical data of patients with or without heterozygous deficiency ofα 1 -ACT were analyzed.Results The level ofα 1 -ACT was0.264±0.055in the asthmatic group with0.265±0.045in the controls.The difference was not significant(t=0.022,P>0.05).Seven of the patients,one of the healthy children and one of the healthy adults were found to have heterozygous deficiency ofα 1 -ACT.The rate of heterozygous deficiency ofα 1 -ACT was14%(7/50)in the asthmatic group,2%(1/50)in the healthy childhood control group,and1%(1/100)in the healthy adult control group.The rate of heterozygous deficiency ofα 1 -ACT in the parents of the asthmatic patients was much higher(38%,7/18)than that in adult controls(1%).This difference was statistically significant(χ 2 =28.914,P<0.01).The rate of heterozygous deficiency ofα 1 -ACT in the asthmatic group was much higher than in the adult control group(χ 2 =8.731, P<0.05),and also much higher than that in the childhood control group(χ 2 =4.891,P<0.05).Besides,the rate of heterozygous deficiency ofα 1 -ACT in the asthmatic group in Guangzhou was higher than that in Chongqing(χ 2 =4.054,P<0.05).The number of hospitalization resulting from asthma was higher for those with heterozygous deficiency ofα 1 -ACT in comparison with those patients without heterozygous deficiency ofα 1 -ACT.In addition,the rate of being RAST positive was also strongly elevated in those with heterozygo- sity.Conclusions These findings suggested that there were close association between heterozygous defiˉciency ofα 1 -ACT and childhood asthma.Geography also affects the distri
