摘 要:目的 探讨彩色多普勒超声鉴别诊断乳腺肿瘤的价值。方法 对 71例经彩色多普勒超声诊断的乳腺肿瘤患者进行回顾性分析 ,根据手术病理结果将患者分成乳腺癌及乳腺纤维腺瘤两组 ,对比分析它们的二维声像图 ( 2 DUS)以及彩色多普勒血流显像 ( CDFI)的特征。结果 1 .乳腺良恶性肿瘤的血流信号检出率无明显差异 (恶性组 1 0 0 % ,良性组 85 % ,P>0 .0 5 ) ;2 .乳腺恶性肿瘤的血流信号丰富程度、收缩期峰值流速 ( Vmax)、阻力指数 ( RI)、搏动指数 ( PI)均高于良性肿瘤 ,良、恶性间有显著性差异 ( P<0 .0 5 ) ;3.我们以血流信号丰富程度 II级或III级、Vmax>0 .2 5 m/s、RI>0 .7、PI>1 .4为恶性肿瘤诊断标准 ,并提出以下诊断方法 :如果二维超声诊断为恶性且上述四项指标中有两项或以上达到标准 ,则应高度怀疑为乳腺癌 ;如二维超声诊断为良性但上述指标有三项或以上达到标准 ,也应考虑乳腺癌可能。应用上述方法诊断乳腺肿瘤的准确性、敏感性、特异性分别为 90 %、89%、91 %。Objective To assess the value of color Doppler ultrasound(CDU)in distinguishing breast tumors.Methods Seventy one patients diagnosed as breast tumors using CDU underwent analysis retrospectively.Patients were divided into malignant group(breast cancer)and benign group(breast fibroadenomas)and they were compared with the characteristics on 2 dimenslonal ultrasound and color Doppler. Results (1)The detection rate of blood flow signal showed no differences between the two groups.(benign group 85%;malignant group 100%, P >0 05).(2)In malignant group,the degree of blood abundance,peak systolic velocity(Vmax),RI and PI were all significantly higher than those in benign group( P <0 01).(3)The criteria for diagnosing malignant tumors were as follows:The degree of blood abundance was Ⅱ or Ⅲ grades,Vmax>0 25 m/s,RI>0 7;and PI>1 4.The diagnostic method was that,if the tumor was diagnosed as malignant tumor in 2 D ultrasound,and 2 out of 4 indices listed above met the criteria,then it should be highly suspected as a cancer.If the tumor was first diagnosed as benign tumor in 2 D ultrasound,but 3 indices met the standard,then it should also be considered as a cancer.The accuracy,sensitivity and specificity were 90%,89% and 91% respectively.Conclusions Color Doppler ultraonography being used as a noninvasive method for detecting breast tumors has wild applicable prospect in differentianting breast tumors.
