双吻合术在低位直肠癌117例保肛手术中的应用评价

摘　　要：目的　评价双吻合术在低位直肠癌保肛手术中应用的优越性。方法　回顾性分析我院 1 997年开始采用双吻合术行低位直肠癌保肛手术以来的 1 1 7例的经验。结果　所有病例应用双吻合器进行直肠闭合和吻合进程顺利 ,术后发生吻合口漏 3例 (2 .6 % ,3/ 1 1 7) ,吻合口狭窄 2例 (1 .7% ,2 / 1 1 7) ,吻合口出血 4例 (3.4 % ,4 / 1 1 7) ,切口感染 2例 (1 .7% ,2 / 1 1 7) ,术后肛门疼痛 1例 (0 .9% ,1 / 1 1 7) ,无手术死亡。结论　双吻合术能使外科医生顺利完成以往手法吻合不易或难以完成的低位直肠吻合 。Objective To evaluate the superiority of double stapling technique(DST)in anus saving operations for patients with low rectal carcinoma.Methods DST was used in 117 patients with low rectal caicinoma from 1997 to 2002,and the results were evaluated.Results Operation of all the 117 patients was successful.Complications included anastomotic leakage in 3 patients(2.6%,3/117),anastomotic stenosis in 2 patients(1.7%,2/117),anastomotic bleeding in 4 patients(3.4%,4/117),wound infection in 2 patients (1.7%,2/117),and anal pain in 1(0.9%,1/117),no death.Conclusion DST can help surgeon to realize low colorectal anastomosis which would otherwise be difficult or impossible with traditional hand sewn method.DST provide a safe alternative for anus saving operation in patients with low rectal carcinoma.
