膝关节周围巨大骨巨细胞瘤的保肢治疗:对患者预后影响9例分析

摘　　要：目的:采用人工膝关节治疗膝关节周围巨大骨巨细胞瘤,选择手术适应证并观察患者肢体功能保留效果。方法:9例患者,良性骨巨细胞瘤6例,恶性3例。均采用骨瘤段切除,双关节半限制型人工膝关节置换8例,旋转限制型人工交链膝关节置换1例。结果:随访1.5～4.7年,1例死于肺部转移,8例无瘤存活。8例患者术后获得了较好的膝关节功能及生活质量,假体无松动。结论:人工膝关节置换是治疗巨大骨巨细胞瘤的有效方法,可以早期锻炼膝关节的功能,减少传统手术所带来的术后复发及关节病变。AIM:To choose the surgical operation indication and observe the reserve effect of the patients' limb function by using artificial knee joint to treat the enormous giant cell tumor of bone (GCT) around the knee joint. METHODS:Nine cases were chosen including 6 cases of benign GCT, 3 of malignant GCT.Bone tumor resection was adopted, and artificial knee joints replaced examples pairs of joint restriction types in 8 cases,rotate artificial to pay chain replaced examples by knee joint restriction type 1 case. RESULTS:Follows up 1.5-4.7 years,1 case died of the lung to shift, 8 cases had no tumour survived.Eight cases had gotten better knee joint function and life quality after operation, artificial knee joints had not become flexible. CONCLUSION:It is an effective method to treat enormous giant cell tumor of bone with artificial knee joint replace method, practise the functions of knee joint in earlier period, and reduce recur and the joint disease leaded by the traditional operation.
