急性白血病患者周期素Cyclin D_1蛋白的测定及其临床意义

摘　　要：目的　研究急性白血病患者周期素CyclinD1 蛋白的表达情况并结合临床分析其意义。方法　采用免疫组化SP法 ,对 4 8例急性白血病患者的骨髓进行CyclinD1 蛋白测定 ,其中急性非淋巴细胞白血病 (ANLL) 32例 ,急性淋巴细胞白血病 (ALL) 1 6例 ,男 31例 ,女 1 7例 ,年龄 1 2～ 70岁 ,平均 34 4岁 ,另有 5例正常人骨髓。结果　正常人骨髓未见阳性染色。患者阳性率为 31 3% (1 5 /4 8) ,其中ANLL和ALL的阳性率分别为 2 5 0 % (8/32 )和 4 3 8% (7/1 6 ) ,两者差异无统计学意义。初治患者的阳性程度多为轻中度 ,而复发患者的阳性程度则多数较高。结论　CyclinD1 蛋白在急性白血病患者中存在过度表达的情况 ,其表达程度与临床病情进展程度有一定的关系 。Objective To determine the expression of Cyclin D 1 protein in patients with acute leukemia, and to evaluate the relationship between Cyclin D 1 expression and clinical treatment outcomes. Methods Cyclin D 1 protein was examined by immunohistochemical method in 48 acute leukemia patients, among whom 32 were ANLL and 16 were ALL, male 31, female 17, aging from 12 to 70, years with a mean of 34 4 years. It was also assayed in five normal bone marrow samples. Results There is no Cyclin D 1 protein detected in normal bone marrow cells. The positive rate of this protein in the patients is 31 3%(15/48), 25 0%(8/32)in ANLL and 43 8%(7/16)in ALL, respectively. There was no significant difference between the positive rate of the patients with ANLL and that with ALL. The positive rate of Cyclin D 1 protein was highest in the relapse group and lowest in the CR group, and the expression degree was higher in the relapse group than that in patients at the time of diagnosis. Conclusion Overexpression of Cyclin D 1 does exist in patients with leukemia, and its expression degree is correlated with progression of the disease, especially with relapse. Cyclin D 1 protein may be a prognostic factor relating to poor outcome in our study
