原发与术后复发髓外硬膜下肿瘤24例分析

摘　　要：目的　了解髓外硬膜下肿瘤原发与术后复发病灶的形态特征差别及其低场磁共振的诊断价值。方法　2 4例髓外硬膜下肿瘤行低场磁共振检查 ,原发病例 17例 ,术后复发病例 7例。磁场强度 0 .0 64T。其中有 6例另行GD-DTPA增强扫描。全部病例经手术病理证实。结果　 ( 1)原发神经鞘瘤、神经纤维瘤和脊膜瘤的部位、形态、磁共振信号及强化程度均与高场磁共振表现相仿。 ( 2 )三种肿瘤术后复发病灶在硬膜下生长的同时常跨越椎间孔向外生长 ,病灶明显比原发灶大。 ( 3 )复发灶形态特征与原发灶相比发生较大变化 ,神经鞘瘤呈巨块分叶形或圆柱形 ,脊膜瘤呈管形 ,神经纤维瘤呈哑铃形。结论　低场磁共振能清晰显示原发与术后复发髓外硬膜下肿瘤的部位 ,形态特征 ,对确定诊断有重要价值。Objective To analyze the different characteristics between primary and recurrent intradural extramedullary tumors(IEMT)and make evaluation of low field MRI in diagnosing them.Methods 24 cases of IEMT were examined by low field MRI,of which 17 cases were primary tumors and 7 cases recurrent tumors postoperatively.All cases were proved by operation and pathology.Results (1)the appearance of IEMT in MRI with low field is the same as that in MRI with high field.(2)the recurrent tumors are often larger than primary ones,growing out of foramen intervertebrale.The characteristics are different between primary tumors and recurrent tumors.For recurrent tumors,lobular or cylinder in facicular neuroma,dumbbell-shaped in neurofibroma and tube-shaped in menigoma.Conclusion MRI with low field can still show the characteristics and location of intradural extramedullary tumors clearly,whether primary or recurrent tumors.
