氩氦刀联合放疗治疗巨大原发灶非小细胞肺癌初探

摘　　要：目的初步探讨氩氦刀联合放疗治疗非小细胞肺癌的局部疗效。方法 2 0 0 1年 1 1月至 2 0 0 2年 7月治疗 5例局部晚期非小细胞肺癌 ,中位年龄 6 7岁 ;周围型 4例 ,中央型 1例 ,原发肿瘤最长径为 6～ 1 0cm ,KPS3R 70。先采用氩氦冷冻经皮穿刺治疗。 1～ 2周后开始放疗 ,TD6 5～ 75Gy/ 6 .5～ 7.5w。氩氦刀治疗后 1～ 2周评价其疗效。氩氦刀 +放疗疗效 (WHO标准 )在疗程结束后评价。结果 1例CR ,4例PR ,总有效率为 1 0 0 %。并发症 :2例血痰 ,1例术中渗血 ,对症支持治疗后愈。随访 3～ 1 8月 ,所有患者切口处无肿瘤种植 ,治疗的第一例随访 1 8个月继续存活。结论巨大原发病灶非小细胞肺癌采用氩氦刀结合放疗是一种有效、微创。purpose To evaluate the effect with cryosurgery plus radiotherapy for non-small-cell lung cancer(NSCLC).Methods and material: Five patients with locally advanced NSCLC were treated in Dec.2001 to Jul. 2002.The median age was 67. Of the five cases, the type of central and epitheal was four and one respectively and the size of primary tumor was 6-12cm. KPS≥70. The patients were treated by cryosurgery firstly and by radiotherapy after 1-2 weeks. The total dose was 65-75Gy/6.5-7.5w. The response of post-cryosurgery was evaluated after 1-2 weeks. The overall response rate was evaluated that cryosurgery and radiotherapy were finished.Results One patient was complete response. Two patients were partial response. The response rate was 100%. Three patients experienced the complications that included two incidences of hemoptysis and one incidence of bleed from the vagina. The incidences can be treated. Conclusion It is an safe, effective regimen for the large primary tumor of non-small-cell lung cancer with cryosurgery plus radiotherapy.
