不同酶抑制剂对检测肠杆菌科细菌超广谱β-内酰胺酶敏感性探讨

摘  要:目的 比较不同酶抑制剂检测肠杆菌科细菌超广谱 β -内酰胺酶 (ESBLs)敏感性。  方法 分别以克拉维酸 (CA)、他唑巴坦 (TA)、舒巴坦 (SU)为酶抑制剂 ,氨曲南 (AZT)、头孢他啶 (CAZ)、头孢噻肟 (CTX)为底物 ,用双纸片协同法检测 188株肠杆菌科细菌ESBLs。 结果  188株肠杆菌科细菌ESBLs总检出率为 41.5 % ,其中以CA、TA、SU为酶抑制剂ESBLs检出率分别为 3 7.8%、2 2 .9%、3 4.0 % ;以CTX、AZT、CAZ为底物ESBLs敏感性分别为 82 .0 %、64 .2 %、5 2 .2 %。 结论 检测产ESBLs肠杆菌科细菌以克拉维酸为酶抑制剂检出率最高 ,以头孢噻肟为底物最敏感。利用不同酶抑制剂联合不同底物做双纸片协同试验 ,可以提高肠杆菌科细菌产ESBLs菌株检出率。Objective To compare the sensitivity for detection of ESBLs by differtent enzyme-inhibitors. Methods double-disk synergy test was used to detect ESBLs in 188 strains of the family enterobacterianceae. Results 41.5% of strains were considered ESBLs producers by double-disk synergy test. Using CA、TA、SU , 37.8%, 22.9%, 34.0% of the family enterobacterianceae are detected out respectively. The sensitivity for detection of ESBLs by differtent enzyme-inhibitors in combination with CTX、AZT、CAZ is 82.0%, 64.2%, 52.2%. Conclusions CA was the most sensitive enzyme-inhibitors and CTX was the most sensitive indicator inscreening ESBLs producing the family enterobacterianceae. ESBLs producers are easier to be detected out by double-disk synergy test using differtent enzyme-inhibitors in combination with varied antimicrobial disk.
