胃癌及癌前病变中树突状细胞浸润、HLA-DR抗原表达的临床意义

摘　　要：目的　探讨S 10 0 + DC浸润、HLA DR抗原的表达在胃癌发生、发展中的作用及临床意义。方法　应用免疫组织化学方法对 4 3例胃癌、2 4例癌前病变组织中的S 10 0 + DC、HLA DR进行检测。结果　S 10 0 + DC、HLA DR在胃癌组、癌前病变组中的表达均显著高于正常胃粘膜组 (P <0 .0 5 )。在低分化、淋巴结转移和远处转移胃癌患者中S 10 0 + DC数目明显减少 (P <0 .0 1)。HLA DR在低分化胃癌中的阳性表达率显著降低 (P <0 .0 5 )。S 10 0 + DC与HLA DR的表达有正相关性。结论　胃粘膜中S 10 0 + DC与HLA DR的变化反映了胃癌局部免疫状态 ,二者在胃癌发生、发展中起重要作用。Objective To study the clinical significance and effects of S-100 + DC infiltration and the expression of HLA-DR antigen in carcinogenesis and progression of gastric carcinoma. Methods S-100 + DC and HLA-DR antigen were detected in 43 specimens of gastric carcinoma,24 cases of precancerous lesions by immunohistochemical methods.Results The expression of S-100 + DC and HLA-DR in gastric carcinoma,precancerous lesions was markly higher than in normal gastric mucosa ( P < 0.05 ). The numbers of S-100 + DC in the gastric carcinoma patients with poor differentiation,lymph node and distant metastasis were significantly lower ( P < 0.01 ). The positive rates of HLA-DR in the patients with poor differentiated gastric carcinoma was significantly lower ( P < 0.05 ). A positive relationship was found between S-100 + DC and HLA-DR.Conclusion The change of S-100 + DC and HLA-DR in gastric mucosa play an important role in gastric carcinogenesis.They can reflect the local immune status of gastric carcinoma.
