辨证分型针刺治疗血液流变学异常偏头痛162例临床研究

摘　　要：目的　观察辨证分型针刺治疗血液流变学异常偏头痛的临床疗效及治疗前后血液流变学指标的变化。方法　1 62例血液流变学异常偏头痛患者辨证分为 5型取穴针刺治疗 ,分别于针刺治疗前后进行症状学和检验学总评分及血液流变学指标测定。结果　临床总有效率 93 .2 %;针刺治疗前后症状学积分分别为 ( 1 3 .2 3±3 .2 5 )分和( 9.1 2± 2 .48)分 ,检验学积分分别为 ( 6.5 7± 3 .89)分和 ( 4 .93± 3 .1 7)分 ,治疗前后比较均有显著性差异 (P <0 .0 1或P <0 .0 5 ) ;血液流变学指标全血高切黏度、全血低切黏度、血浆比黏度、血浆纤维蛋白原治疗后比治疗前均明显降低 (P <0 .0 5或P <0 .0 1 )。结论　辨证分型针刺治疗血液流变学异常偏头痛疗效确切 ,且针刺治疗能改善血液流变学的多项指标 ,提示偏头痛患者血液流变学改变可能为偏头痛病理改变的构成因素。Objective To observe the therapeutic effect of acupuncture therapy according to TCM differentiation of syndromes on migraine due to hemorheological abnormality and the changes of hemorheological parameters before and after treatment.Methods 162 patients with migraine due to hemorheological abnormality were selected the points to be acupunctured according to the TCM differentiation of syndromes (5 types). The clinical symptom score, docimastic score and hemorheological parameters were observed and compared before and after treatment.Results The total effective rate was 93.2%;clinical symptom scores before and after treatment were 13.23±3.25 and 9.12±2.48 respectively, there was a significant difference before and after treatment (P<0.01);the docimastic scores before and after treatment were 6.57±3.89 and 4.93±3.17 respectively, there was also a significant difference before and after treatment (P<0.05);hemorheological parameters after treatment were significantly decreased, as compared with those before treatment (P<0.05 or P<0.01).Conclusion The acupuncture therapy according to TCM differentiation of syndromes is effective in treating migraine due to hemorheological abnormality, which can improve the hemorheological parameters, which suggests that hemorheological abnormality may be the main pathological changes in patients with migraine.
