p16 CyclinD_1 C-erbB_2在乳腺癌中的表达及临床意义

摘　　要：目的 :探讨乳腺癌组织分型、分级等临床病理特征和ER、PR与 p16、CyclinD1、C -erbB2 蛋白表达的关系。方法 :进行免疫组化S—P染色法 ,检测 5 8例术前均未做放、化疗的乳腺癌组织中 p16、CyclinD1、C -erbB2 蛋白的表达。结果 :p16蛋白表达阳性率为 6 7 2 4 % (39/5 8) ,在乳腺癌Ⅱ级与Ⅲ级之间 p16蛋白阳性率分别为 76 7% (2 3/30 )和 33 33% (4 /12 ) ,差异有显著性 (P <0 0 5 ) ;原发癌伴淋巴结转移组 p16的阳性表达率 5 5 17% (16 /2 9) ,低于不伴淋巴结转移组 p16的阳性率 79 32 % (2 3/2 9) ,差异有显著性 (P <0 0 5 ) ,p16表达与肿瘤大小、ER、PR无相关性。CyclinD1蛋白表达阳性为 6 3 79% (37/5 8) ,乳腺癌Ⅱ、Ⅲ级之间CyclinD1蛋白表达差异有显著性 (P <0 0 5 ) ;CyclinD1与病理分型、肿瘤大小、淋巴结转移、PR未见相关 ,与ER正相关。C -erbB2 蛋白表达阳性率 37 92 % (2 2 /5 8) ,C -erbB2 癌基因表达与腋淋巴结转移和乳腺癌组织分级相关 (P <0 0 1) ,与ER、PR肿瘤大小、分型无相关。结论 :①p16、CyclinD1、C -erbB2 基因蛋白异常与乳腺癌发生有关 ;②p16、C -erbB2 与淋巴结转移有关 ;③p16、CyclinD1、C -erbB2 蛋白可作为判断乳腺癌预后的参数 ;④p16、CyclinD1、C -erbB2Objective:To investigate the clinical pathological features of histological typing and grading and the correlationship between ER,PR and the expression of p16,CyclinD 1, and C-erbB 2 proteins.Method:The expression of p16 CyclinD 1 and C-erbB 2 was detected by immunohistochemical method(S-P staining) in 58 cases of mammary cancers without preoperative radiotherapy and chemotherapy.Results:The positive expression rates in grade Ⅱ and Ⅲ of mammary cancer was respectively 76.7% and 33.33%,Showing significant difference(P<0.05).The positive expression of p16 in primary cancer with lymphatic metastasis was much lower than that without lymphatic metastasis (55.17% versus 79.32%,P<0.05),p16 expression was not correlated with tumor size,ER and PR.The positive expression rate of cyclinD 1 protein was 63.79%,showing significant difference between grade Ⅱ and Ⅲ in mammary cancer (P<0.05).It was not correlated with pathological typing,tumor size and lymphatic metastasis,but correlated with ER.The positive expression rate of C-erbB 2 was 37.92%.C-erbB 2 oncogene expression was correlated with axillary lymph node metastasis and histological grading of mammary cancer (P<0.01),but not correlated with ER,PR,tumor size and typing.Conclusion:①Abnormal p16,cyclnD 1,C-erbB 2 gene proteins were correlated with mammary cancer.②p16,C-erbB 2 were correlated with lymph node metastasis.③p16,CyclinD 1,and C-erbB 2 protein may be predictors for prognosis of mammary cancer.④There is no correlationship among p16,C-erbB 2 and CyclinB 1.They play roles respectively.
