摘　　要：目的　观察Ⅱc型早期胃癌内镜下形态、部位和病理特点 ,探讨其对凹陷性病变的诊断价值。方法　比较 30例Ⅱc型早期胃癌的内镜下形态特点 ,对其中 1 9例内镜下活检和术后病理类型、浸润深度进行对比。结果　Ⅱc型早期胃癌分化型以凹陷发红伴隆起多见 ;未分化型的凹陷表面褪色和结节多。内镜下活检与术后病理类型、浸润深度符合率在 70 %以上。Objective To study Ⅱc type gastric cancer with regard to endoscopic presentation,location,pathological characteristics,pathological type from endoscopic biopsies and post operative samples depth of infiltration and,in order to enhance the understanding and diagnosing of concave lesion.Methods Endoscopic appearances among 30 patients with early stage Ⅱc type gastric cancer were compared.The pathological types and depth of infiltration from endoscopic biopsies and post operative samples of 19 cases were compared.Results The differentiated Ⅱc type gastric cancer in the early stage shows morphological difference macroscopically from the undifferentinted ones.Most differentiated cancers were concave,reddish and protuberant,while undiffercntinted cancers were pale nodules in the concavity.The consistency between biopsies and postoperative pathology reports exceeds 70%with respect to depth of infiltration.Conclusions Early stage Ⅱc type gastric cancer shows morphological characteristics which can identify tentatively the differentiated and the undifferentiated cancers.Both endoscopic diagnosis and biopsy are useful in guiding clinical management.
