摘　　要：目的 :了解上海农村地区癫患病率和治疗缺口 ,为开展癫治疗和管理工作提供依据。方法 :以二阶段整群抽样法抽取金山区吕巷镇的 5个村作为调查点 ,实际调查 10 777人。癫病例经初筛及神经科医生复查后确诊。结果 :确诊癫为 65例 ,终生患病率 6 0 3‰ ( 95 %CI :5 5 8‰～ 6 48‰ )。活动性癫为 41例 ,患病率为 3 80‰ ( 95 %CI :2 64‰～ 4 96‰ )。活动性癫的治疗缺口为70 73 % ( 95 %CI:5 6 80 %～ 84 66% )。癫患者平均年龄为 ( 4 4 66± 18 73 )岁 ,首次发病的平均年龄为 ( 2 6 2 8± 18 71)岁 ,首发年龄在 3 0岁之前的患者占 61 5 4%。癫的发作类型以全身强直 阵挛性发作为主 ,占 5 8 46%。结论 :农村地区癫患病率高于以往的调查结果 ,应重视对农村儿童和青年期癫的治疗和管理。Aim:To investigate the prevalence of epilepsy in Shanghai rural area and to provide the evidence for the treatment and administration of epilepsy.Methods:The stratified cluster sampling was used for the investigation of 10 777 population in 5 villages of Luxiang Country, Jinshan District. All the epilepsy patients were diagnosed by screening and examinations of neurologists. Results:Sixty-five epilepsy cases were diagnosed, and the lifetime prevalence was 6.03‰ (95%CI:5.58‰～6.48‰).Forty-one cases were diagnosed as active epilepsy,and the prevalence was 3.80‰(95%CI:2.64‰～4.96‰). The treatment gap of the active epilepsy was 70.73%(95%CI:56.80‰～84.66%). The average age of the epilepsy cases was 44.66±18.73 years old, and the average age of first seizure occurrence was 26.28±18.71 years old. Sixty one point fifty-four percent of cases had their first seizure before 30 years old. The generalized tonic-clonic seizure (GTC) was the common epilepsy type that was found within 58.46% epilepsy cases. Conclusion:The prevalence data from this study was higher than in previous experience. It is important to emphasize the treatment and administration of epilepsy patients of children and teenagers in rural area.
