摘 要:目的:本研究旨在分析结直肠高危腺瘤的风险因素,以及与血清胆红素水平的关系。方法:本研究纳入2017年5月至2022年6月在青岛大学附属医院接受结肠镜检查的415例患者。采用单因素分析研究结直肠高危腺瘤风险因素,多因素logistic回归进一步分析结直肠高危腺瘤与血清胆红素的关系。结果:高危腺瘤组中男性、结直肠癌家族史患者更多。与非高危腺瘤组相比,高危腺瘤组患者平均年龄、血清直接胆红素(DBIL)、中性粒细胞/淋巴细胞比值(NLR)更高,总胆固醇(TC)更低。血清直接胆红素与结直肠高危腺瘤独立相关。结论:性别、年龄、结直肠癌家族史、直接胆红素、中性粒细胞/淋巴细胞比值、总胆固醇与结直肠高危腺瘤相关。血清直接胆红素水平是结直肠高危腺瘤的独立风险因素。Objective: The purpose of this study was to analyze risk factors of advanced colorectal adenoma and the association between advanced colorectal adenoma and serum bilirubin. Methods: A total of 415 patients who underwent colonoscopy in the Affiliated Hospital of Qingdao University from May 2017 to June 2022 were included in this study. Univariate analysis was used to study the risk factors of advanced colorectal adenoma, and multivariate logistic regression was used to further analyze the association between advanced colorectal adenoma and serum bilirubin. Results: The advanced col-orectal adenoma group had more patients with male sex and a family history of colorectal cancer. The mean age, serum direct bilirubin (DBIL), neutrophil/lymphocyte ratio (NLR) of patients with advanced colorectal adenoma was higher than those without. Patients with advanced colorectal adenoma had lower total cholesterol (TC). Serum direct bilirubin was independently associated with advanced colorectal adenoma. Conclusions: Gender, age, a family history of colorectal cancer, serum direct bilirubin, neutrophil/lymphocyte ratio and total cholesterol are associated with ad-vanced colorectal adenoma. Serum direct bilirubin level is an independent risk factor for advanced colorectal adenoma.
