集合不足型外隐斜对近视儿童眼轴增长的影响

摘  要:目的:探讨集合不足型外隐斜与近视儿童眼轴增长的关联。方法:队列研究。选取2020年2月~2021年12月就诊于山东大学齐鲁医院(青岛) 6~16岁近视儿童152例参与调查,其中集合不足型外隐斜82例,眼位正位70例。基线时行眼部检查、眼轴测量,1年后复测眼轴并行问卷调查。采用多元线性回归分析集合不足型外隐斜与近视儿童眼轴增长的关联。结果:在多元线性回归分析中,调整父母近视人数、年龄、性别、每天课外近距离用眼时间、每天户外活动时间后,眼位(β = 0.054, 95%CI: 0.021~0.087)与近视儿童眼轴增长存在显著关联。集合不足型外隐斜近视儿童眼轴增长量M (QR)为0.37 (0.26) mm,显著高于眼位正位组的0.29 (0.15) mm,差异有统计学意义(P = 0.009)。结论:集合不足型外隐斜与近视儿童眼轴增长存在关联,更容易导致近视儿童眼轴增长。Objective: To explore the correlation between convergence insufficiency exophoria and axial length growth in children with myopia. Methods: Cohort study. A total of 152 myopic children aged 6 to 16 years old who were admitted to Qilu Hospital (Qingdao), Cheeloo College of Medicine, Shandong University from February 2020 to December 2021 were selected to participate in the investigation, including 82 cases of convergence insufficiency exophoria and 70 cases of orthophoria. At baseline, eye examination and axial length measurement were performed, and one year later, the axial length measurement was repeated and a questionnaire survey was conducted. Multiple linear re-gression was used to analyze the association between convergence insufficiency exophoria and axial length growth in children with myopia. Results: In multiple linear regression analysis, after adjust-ing for the number of parents with myopia, age, gender, time spent on using eyes at close range out-side class every day and time spent on outdoor activities every day, ocular alignment (β = 0.054, 95%CI: 0.021~0.087) was significantly associated with axial length growth in children with myopia. The axial length increase M (QR) was 0.37 (0.26) mm, which was significantly larger than that of the orthophoria group (0.29 (0.15) mm), the difference was statistically significant (P = 0.009). Conclu-sions: Convergence insufficiency exophoria is associated with axial length growth in myopic children, and it is more likely to cause axial length growth in myopic children.
