血清寡糖链检测在乙肝相关性肝癌中的诊断价值

摘　　要：目的:探究血清寡糖链检测(G-test)在乙肝相关性肝癌(Hepatocellular Carcinma, HCC)中的诊断价值。方法:收集2019年8月至2019年10月就诊于空军军医大学第二附属医院的患者和健康体检者共410例,其中HCC 41例,治疗后HCC 49例,慢性乙型肝炎68例,慢性乙型肝炎合并肝囊肿7例,慢性乙型肝炎合并血管瘤6例,乙肝肝硬化69例,其他恶性肿瘤73例,健康人群97例,检测其血清G-test值、甲胎蛋白(Alpha Fetal Protein, AFP)含量,统计分析并比较G-test、AFP分别诊断HCC的灵敏度、特异度、总符合率、约登指数、受试者工作特征(Receiver Operator Characteristic, ROC)、曲线下面积(Area under of the curve, AUC)。结果:G-test与年龄、性别无相关性,G-test在HCC中的灵敏度、总符合率、约登指数、ROC曲线下面积均高于AFP。结论:G-test较AFP在原发性肝癌中具有更好的诊断价值。Objective: To explore the diagnostic value of serum G-test in hepatocellular carcinoma. Methods: 410 cases in the Second Affiliated Hospital of Air Force Medical University from Aug. 2019 to Oct. 2019 were selected, including hepatocellular carcinma (HCC) team (41 cases), Post-treated HCC team (49 cases), chronic hepatitis B (CHB) team (68 cases), CHB complicating with hepatic cyst (CHB & HC) team (7 cases), CHB complicating with hepatic hemangioma (CHB & HH) team (6 cases), hepa-titis B cirrhosis (HBC) team (69 cases), other cancers team (73 cases) and health team (97 cases). G-test and alpha fetal Protein (AFP) were tested in every cases. The sensitivity, specificity, crude agreement rate, Youden index, likelihood ratio and AUC were statically analyzed. Results: The sen-sitivity, crude agreement rate, Youden index, AUC of G-test were larger than those of AFP. Conclu-sions: G-test had a better diagnostic value in hepatocellular carcinoma than AFP.
