胸腔镜下肺癌根治术患者实施肺康复干预阻碍因素的质性研究

摘　　要：目的:从患者的视角探讨将肺康复纳入常规肺癌临床治疗的阻碍因素,为制定全面的有针对性的临床实践干预提供参考依据。方法:采用现象学研究的方法,对15名胸腔镜下肺癌根治术患者进行半结构式深入访谈,并应用Colaizzi内容分析法分析资料。结果:胸腔镜下肺癌根治术患者实施肺康复干预阻碍的因素可归纳为5个主题:知识、信念和医护人员提供信息的重要性;症状负担;营养失调;体重下降;社会支持和环境。结论:肺癌根治术患者实施肺康复的阻碍因素是多方面的,医护人员应深入了解肺癌患者的身心感受,增加肺癌患者肺康复知识教育,提供常见症状的管理措施,加大社会支持力度和改善患者运动环境,为肺癌患者提供一个精细的、以肺癌患者为中心的针对性的肺康复计划,以促进肺康复的实施,促进患者的快速康复。Objective: From the perspective of patients,the obstacles of including lung rehabilitation in routine clinical treatment oflung cancer were discussed, so as to provide a reference for the development ofcomprehensive and targeted clinical practice intervention. Methods: In this study,15 patients underwent radical thoracoscopic lung cancer were interviewed by phenomenologicalmethod, and the data were analyzed by Colaizzi content analysis method. Results:The factors that hinder the implementation of pulmonary rehabilitation interventionin patients undergoing thoracoscopic radical resection of lung cancer can be summarizedinto five themes: knowledge, belief, and the importance of information providedby medical staff;Symptom burden;Malnutrition;Weight loss;Social support andenvironment. Conclusion: Implementation of pulmonary rehabilitation in patientswith lung cancer radical inhibitors are various, medical staff should have a thoroughunderstanding of lung cancer patients feeling of body and mind, increase in lungcancer patients with pulmonary rehabilitation knowledge education, provide a commonsymptom of management measures, strengthen social support and improve patient movementenvironment, provides patients with lung cancer with a delicate, centered on lungcancer patients of pulmonary rehabilitation program,in order to promote the implementation of the pulmonary rehabilitation, promotethe rapid recovery of the patients.
