中医药调控Th17/Treg细胞平衡在类风湿性关节炎中的机制探讨

摘　　要：类风湿性关节炎是临床常见的累及关节及周围软组织的一种慢性自身免疫性疾病,其发病与自身免疫反应密切相关,主要依附于辅助性T细胞(Th17)及调节性T细胞(Treg)失衡,机体免疫稳态失调所致,因此,调节Th17/Treg平衡逐渐成为临床防治RA的新靶点。文章旨在探讨近五年来单味中药有效成分、中药复方及中医外治在恢复Th17/Treg细胞平衡的作用机制,总结其通过调节炎症因子生成、干预信号通路等发挥的治疗作用,以期为临床采用中医药作为RA替代疗法提供坚实的理论依据。Rheumatoid arthritis (RA) is a common chronic autoimmune disease affecting joints and surrounding soft tissues. Its pathogenesis is closely related to autoimmune response, mainly dependent on the imbalance of helper T cells (Th17) and regulatory T cells (Treg), and the imbalance of immune homeostasis. Therefore, regulating the Th17/Treg balance has gradually become a new target for the clinical prevention and treatment of RA. This article aims to explore the mechanism of single TCM active ingredients, TCM compound, and external treatment in restoring the balance of Th17/Treg cells in the past five years and summarize its therapeutic effects by regulating the production of inflammatory factors and interfering with signaling pathways, so as to provide a solid theoretical basis for clinical application of TCM as an alternative therapy for RA.
