摘 要:目的 评价国产纯钛螺钉式微种植体的支抗效果.方法 选取符合纳入标准的患者22例,先后共植入58枚微种植体,分为早期加力组(45枚)和非早期加力组(13枚),观察早期加力对微种植体稳定性的影响.微种植体植入后拍摄头颅侧位定位片,取研究模.对其中12例尖牙远中移动到位的患者再次拍摄头颅侧位定位片并取研究模,分别以头影测量和模型测量的统计结果评价国产螺旋式微种植体的支抗效果.3及6个月后分别拍摄头颅侧位片,并取研究模,重复上述测量过程,观察微种植体中远期疗效.结果 1)早期加力组微种植体脱落率(6.67%)低于非早期加力组(69.23%)(x2=24.06,P<0.05).2)头影测量显示:微种植体治疗前、后距参考线距离分别为(24.59±4.20)mm及(25.08±4.30)mm,平均近中移动0.48mm(P=0.0043),上颌第一磨牙治疗前、后距参考线距离分别为(26.22±3.24)mm及(26.97±3.19)mm,平均近中移动0.75mm(P=0.0188),尖牙治疗前、后距参考线距离分别为(43.68±4.74)mm及(37.33±3.64)mm,平均远中移动6.35mm(P=0.0126).3)模型测量显示:上颌第一磨牙治疗前后距参考线距离分别为(28.01±7.31)mm及(27.12±7.39)mm,平均近中移动0.89mm (P=0.0287),尖牙治疗前后距参考线距离分别为(2.07±1.25) mm及(7.03±3.52)mm,平均远中移动4.96mm (P=0.0448).4)3及6个月重复测量值与治疗后结果差异无统计学意义(P>0.05).结论 国产螺钉式微种植体支抗效果可靠,能满足临床治疗需求,中远期治疗效果稳定.植入后早期加力,严格选择适应证、植入部位,保持局部卫生以及规范操作程序是提高微种植体稳定性的关键.Objective To evaluate the effectiveness of home-made titanium microscrew implant as anchorage,and to summarize the aspects and resolvent that should be pay attention to in clinical application of home-made titanium microscrew implant.Methods The sample consisted of 22 patients.Totally,58 implants were screwed and 12 ablated.Early-load group consisted of 45 implants and not-early-load group had 13 implants.Patients were investigated and the correlation between stability of implants and early-load was evaluated.Cephalomatric radiograms were taken and study casts were poured after the microscrew implants being screwed for each patient.These work were performed again right after 12 patients` canines moved to the right position distally,three and six months later respectively.Statistical analysis was done in order to evaluate the effectiveness and mid-and long-term efficacy of home-made titanium microscrew implant.Results(1)The ablating rate of early-load group was 6.67%(3/45)while 69.23%(9/13)in not-early-load group(χ~2=24.06,P<0.05).(2)Cephalometrics:the distances from implants to reference line before and after treatment were(24.59±4.20)mm and(25.08±4.30)mm,the implants moved 0.48mm(P=0.0043)averagely in mesial direction.The distances from maxillary first molars to reference line before and after treatment were(26.22±3.24)mm and(26.97±3.19)mm,the maxillary first molars moved 0.75mm(P=0.0188)averagely in mesial direction.The distances from maxillary canines to reference line before and after treatment were(43.68±4.74)mm and(37.33±3.64)mm,the maxillary canines moved 6.35mm(P= 0.0126)averagely in distal direction.(3)Measurement of study casts:the distances from maxillary first molars to reference line before and after treatment were(28.01±7.31)mm and(27.12±7.39)mm,the maxillary first molars moved 0.89mm(P=0.0287)averagely in mesial direction.The distances from maxillary canines to reference line before and after treatment were(2.07±1.25)mm and(7.03±3.52)mm,the maxillary canines moved 4.96mm(P=0.0448)average
