摘 要:目的:探索心内科专科临床药师培训的新方法,为提高临床药学带教水平提供参考。方法:在理论学习和临床实践中,将传统教学法(LBL)与病例导向(CBL)、问题导向(PBL)、文献导向(RISE)教学方法交叉结合应用于基地临床药师学员的培养。理论教学中穿插进行LBL法与CBL/PBL/RISE法。临床实践教学中,前半部分的查房、问诊、药学监护、用药教育、处方点评及不良反应上报以LBL法为主,交叉CBL/PBL/RISE法为辅,病例讨论及参与制定治疗方案则相反;临床实践教学的后半部分全部以CBL/PBL/RISE法为主,辅以LBL法。结果:LBL-CBL/PBL/RISE交叉教学法在调动学员的积极性和主动性后,配合带教老师所提供的知识、方法和思路,有效提高了学员分析病例、发现问题、文献检索、解决问题和参与制定治疗方案的能力。结论:LBL-CBL/PBL/RISE交叉教学法在给予学员系统性临床药学、临床医学知识的同时,兼顾了临床思维和实践能力的培养,能双向提高学员素质和临床药师带教水平,具有很好的教学效果,特别适用于学习内容多而复杂的专科(心内科)。Objective:To explore new methods for training clinical pharmacists in cardiology specialty to provide a reference for improving the level of clinical pharmacy teaching.Methods:In the theoretical study and clinical practice,the traditional teaching method(LBL)is combined with case-oriented(CBL),problem-oriented(PBL)and literature-oriented(RISE)teaching methods,and applied to the training of clinical pharmacists in the base.In the theoretical teaching,the LBL method and the CBL/PBL/RISE methods are interspersed.In clinical practice teaching,the first half of making the rounds of the wards in a hospital,consultation,pharmaceutical monitoring,drug use education,prescription review and adverse reaction reports are mainly based on the LBL method,and supplemented by the CBL/PBL/RISE method,while case discussion and participation in the development of treatment regimens are on the contrary.The latter part of clinical practice teaching is based on the CBL/PBL/RISE method,and supplemented by the LBL method.Results:The LBL-CBL/PBL/RISE cross-teaching method can effectively improve the abilities of analyzing cases,discovering problems,retrieving literature,solving problems and taking part in the formulation of treatment regimens after mobilizing the enthusiasm and initiative of the students and by combining with the knowledge,methods and ideas provided by the teachers.Conclusion:The LBL-CBL/PBL/RISE crossteaching method not only gives students systematic clinical pharmacy and clinical medical knowledge,but also takes into account the cultivation of clinical thinking and practical ability,which can improve the quality of students and the level of teaching of clinical pharmacists,and has a good teaching effect,and is especially suitable for the specialty(cardiology)with many and complex learning contents.
