摘 要:目的探讨在胰腺癌中Smad-泛素化调节因子2(Smurf2)的表达情况、其表达的临床意义,并分析其可能对胰腺癌患者预后的影响。方法选取2011-01-01-2015-12-01辽宁省人民医院普外科治疗的胰腺癌患者的手术切除组织病理标本58例,将58例配对的胰腺癌及癌旁组织Smurf2蛋白表达通过免疫组织化学进行测定,探究临床病理学参数与Smurf2表达之间是否存在关系;再对与12对新鲜胰腺癌配对的癌旁组织mRNA表达情况以及Smurf2蛋白应用实时定量PCR以及蛋白印迹法进行测定;定量分析Smurf2在分化程度不一致的胰腺癌细胞中mRNA以及蛋白的存在情况;在不同细胞系中定量分析Smurf2差异表达对胰腺癌细胞侵袭和迁移能力的影响;通过Log-rank分析Smurf2表达与胰腺癌患者中位生存期的相关性;采用siRNA干扰明确Smurf2对上皮细胞间质化的关键影响,通过细胞侵袭和迁移实验观察Smurf2对胰腺癌细胞系SW1990迁移以及侵袭的影响。结果Smurf2在58例胰腺癌组织中表达(31.03%)低于癌旁组织(53.45%),差异有统计学意义,χ^(2)=5.971,P=0.014;胰腺癌患者中肿瘤直径<3 cm患者中Smurf2阳性表达高于肿瘤直径≥3 cm患者,差异有统计学意义,χ^(2)=5.769,P=0.016;分化程度中低分化患者中Smurf2表达阳性比例患者于高分化患者,差异有统计学意义,χ^(2)=8.798,P=0.003;T_(1)~T_(2)分期患者中Smurf2阳性表达患者比例高于T_(3)~T_(4)期患者,差异有统计学意义,χ^(2)=5.688,P=0.017;通过Log-rank分析Smurf2表达与胰腺癌患者中位生存期的相关性显示Smurf2阳性表达组与阴性表达组,差异有统计学意义,χ^(2)=7.693,P=0.006。Smurf2随着胰腺细胞分化程度降低表达逐步减弱,同时Smurf2干扰通过下调上皮细胞间质转化(EMT)上皮标记蛋白E-钙粘蛋白和上调间质波形蛋白Vimentin促进高分化SW1990细胞侵袭和迁移力。结论Smurf2作为一个抑癌基因,抑制胰腺癌恶性进展,其低表达可能预示Objective To investigate the expression of Smad-ubiquitin regulation factor 2(Smurf2)in pancreatic cancer and its clinical significance,and to analyze its possible effect on the prognosis of pancreatic cancer.Methods 58 patients with pancreatic cancer treated by Department of general surgery in 2011-01-01-2015-12-01 Liaoning Provincial People’s Hospital were selected.The expression of Smurf2 protein in 58 matched pancreatic cancer tissues and adjacent tissues was detected by immunohistochemistry.The relationship between clinicopathological parameters and Smurf2 expression was investigated.Then the expression of mRNA in adjacent tissues of 12 pairs of fresh pancreatic cancer and Smurf2 protein were measured by real-time quantitative PCR and Western blot.The mRNA and protein expression of Smurf2 in pancreatic cancer cells with different degree of differentiation were quantitatively analyzed.The effects of Smurf2 differential expression on invasion and migration of pancreatic cancer cells were quantitatively analyzed in different cell lines.The correlation between Smurf2 expression and the median survival time of patients with pancreatic cancer was analyzed by Log-rank.SiRNA interference was used to clarify the key effects of Smurf2 on epithelial mesenchymal transition,and the effects of Smurf2 on migration and invasion of pancreatic cancer cell line SW1990 were observed through cell invasion and migration assays.Results the expression of Smurf2 in 58 pancreatic cancer tissues(31.03%)was lower than that in adjacent tissues(53.45%),and the difference was statistically significant(χ^(2)=5.971,P=0.014).The expression of Smurf2in pancreatic cancer patients with tumor diameter less than 3cm was higher than that in patients with tumor diameter greater than 3cm,χ^(2)=5.769,P=0.016;The positive rate of Smurf2 expression in patients with low differentiation was higher than that in patients with high differentiation,χ^(2)=8.798,P=0.003;The proportion of patients with Smurf2 positive expression in T_(1-2)stage was higher
