手辅助腹腔镜右半结肠切除术治疗克罗恩病的疗效分析

摘　　要：目的 探讨手辅助腹腔镜技术(HALS)与开腹手术行右半结肠切除术治疗克罗恩病的安全性和效果.方法 回顾性分析2013年1月至2017年1月在中山大学附属第六医院因克罗恩病行右半结肠切除术患者47例,其中行开腹手术(开腹手术组)39例,行HALS手术(HALS手术组)8例.比较2组临床资料、手术时间、术中出血量、造口率、术后并发症发生率、外科住院时间等指标.结果 与开腹手术组比较,HALS手术组的手术时间更短[181.0(156.8,194.5)min 比240.0(210.0,308.0)min],术中出血量更少[48.0(35.3,66.0)ml比76.0(67.0,85.0)ml],术后外科住院天数更短[9.0(8.0,11.5)d比15.0(12.0,22.0)d],组间差异均有统计学意义(均P<0.01).结论 HALS右半结肠切除是治疗克罗恩病安全有效的方法,比开腹手术的手术时间短,出血少,住院时间短.Objective To compare the safety and short-term efficacy between hand-assisted laparoscopic(HALS group)and open right herni-col ectomy(open group)for Crohnf s disease(CD).Methods CD patients from the 6th hospital of Sun Yat-sen university treated with HALS and open right hemi-colectomy between January 2013 and January 2017 were included.Clinical differences between the two groups,such as clinical characteristics,operation time,blood loss,stoma rate,posloperalive complications and recover)'time,were analyzed.Results 8 patients underwent HALS and 39 patients underwent open surgery.No differences in preoperalive clinical characteristics were found between the two groups.Operation time in HALS group[181.0(156.8,194.5)min]was significantly shorter than that in open group[240.0(210.0,308.0)min].HALS group[48.0(35.3,66.0)ml]resulted in a lower blood loss than open group[76.0(67.0,85.0)ml].Postoperative recovery time in HALS group[9.0(8.0,11.5)d]was shorter than that in open group[15.0(12.0,22.0)d].Conclusion HALS right hemi-colectomy is safe and feasible for Crohr/s disease,as it results in lower blood loss,shorter operation time,and postoperative recovery time.
