Safety Analysis of Intravascular Recanalization Therapy Based on Sofia Catheter in Elderly Patients with Acute Ischemic Stroke

摘  要:Objective: to investigate the therapeutic effect of Sofia catheter thrombectomy in elderly patients with acute ischemic stroke. Methods 22: elderly patients with acute ischemic stroke who underwent Sofia catheter thrombectomy (or) combined with stenting in Beijing Chaoyang integrated traditional Chinese and Western medicine emergency rescue center from January 2021 to December 2021 were analyzed retrospectively, including 8 cases of simple Sofia aspiration and 14 cases of combined stent thrombectomy. The general data, clinical characteristics and imaging manifestations of patients were analyzed. The improved thrombolytic test standard for cerebral infarction (mtici) was used to evaluate the patency of blood vessels. Grade 0-2a was occluded, and grade 2b-3 was successful recanalization;The National Institutes of health neurological deficit score (NIHSS) and the modified Rankin score (MRS) were evaluated before and after operation. Results: among the 22 elderly patients, there were 7 male patients and 15 female patients;The patients were 81-94 years old, with an average age of 84.45 ± 3.32 years. The Glasgow Coma Scale (GCS) score before admission was 4-10, with an average score of 7.95 ± 2.40.The blocked sites were basilar artery in 2 cases, bilateral posterior cerebral artery in 1 case, bilateral internal carotid artery in 1 case, right middle cerebral artery in 1 case, right internal carotid artery in 7 cases, left middle cerebral artery in 7 cases and left internal carotid artery in 3 cases. Preoperative intravenous thrombolysis was performed in 9 patients.22 elderly patients were rated by mtici after treatment. It can be seen that there were 2 patients with grade 2A (9.09%), 6 patients with grade 2B (27.27%) and 14 patients with grade 3 (63.64%).There was no significant difference in NIHSS score between patients after treatment and before treatment (18.18 ± 6.82 vs. 18.77 ± 5.98, P > 0.05).Among the 22 patients, 13 (59.09%) died due to complications or family members giving up rescue.
