重组人p53腺病毒注射液联合放疗治疗口腔鳞癌的近期疗效观察

摘　　要：目的评价重组人p53腺病毒(rAd-p53)注射液联合手术、放射线治疗口腔鳞癌的疗效及安全性。方法对自2005年1月至2008年4月,在解放军总医院手术后接受rAd-p53注射液结合放射线治疗的10例舌癌、12例牙龈癌患者及手术后仅接受单纯放疗的12例舌癌、14例牙龈癌患者进行回顾性调查,并对其疗效及不良反应进行定期复查或电话随访,分析rAd-p53注射液结合放射线治疗的疗效及安全性。结果 rAd-p53注射液联合手术、放疗对舌癌、牙龈癌疗效满意,3年局部复发率分别为0%、8.3%,较仅接受放疗的对照组局部复发率(25%、28.6%)明显偏低。除注射药物后发热及注射部位疼痛外,无其他严重不良反应,疗效优于仅接受放射治疗患者。结论 rAd-p53注射液联合手术及放疗治疗舌癌、牙龈癌安全、有效。Objective To evaluate the short-term therapeutic effect and security of recombinant human adenovirus p53 injection(rAd-p53)in combination with operation and radiotherapy in patients suffering from oral SQCA(squamous carcinoma).MethodsA retrospective study of oral SQCA patients that operated in 301 hospital from January 2005 to April 2008 was conducted.Among those patients,10 cases with carcinoma of the tongue and 12 cases with carcimoma of the gingival received extra treatment of rAd-p53,while other 12 patients with tongue carcimoma and 14 patients with gingival carcimoma received only operation and radiotherapy.All patients were reviewed regularly or followed up by telephone.The therapeutic effect was evaluated by the rate of local relapse and living quality,respectively.ResultsThe recurrence rate of the patients treated by radiotherapy and rAd-p53,reached 0%(tongue carcimoma patients)and 8.3%(gingival carcimoma patients),respectively.While the recurrence rate of the patients treated by radiotherapy reached 25%(tongue carcimoma patients)and 28.6%(gingival carcimoma patients),respectively.No other obvious adverse reaction was observed,except for fever and injection site pain.The therapeutic effect of rAd-p53 was obvious,compared with typical radiotherapy.ConclusionsrAd-p53 in combination with operation and radiotherapy is effective and safe for tongue carcimoma or gingival carcinoma patients.
