摘 要:目的对符合培训条件的90名超声医师进行(STEP)介入培训,即筛选(screening)、理论(theory)、考察(examination)、实践(practice)课程培训,以探讨介入性超声医师的培训方法。方法根据培训方式的不同,将90名医师分为培训组、教材组和自学组(每组30名);根据学习进度分为理论学习、模拟操作和实践操作3个阶段,每个阶段学习结束后考核合格的医师进入下个阶段的介入培训学习。结果前两个阶段学习结束后,培训组医师介入培训通过认证的人数(28/28)、通过率100%(28/28)及平均测评分数(84.6±8.0)均高于教材组(16/21,76.2%,69.5±10.0分)和自学组(15/20,75.0%,67.3±13.0),且通过率与教材组和自学组比较差异均有统计学意义(χ2=5.0531,P=0.0246;χ2=5.3645,P=0.0206),而教材组与自学组介入培训认证通过的人数、通过率及平均测评分数的比较差异均无统计学意义(χ2=0.0757,P=0.7833);培训组第3个阶段介入培训通过认证的人数(26/28)、通过率(92.9%)与教材组(14/16,87.5%)和自学组(12/15,80.0%)相比差异均无统计学意义(χ2=0.0025,P=0.9605,χ2=0.5692,P=0.4506);经过3个阶段的学习,培训组的通过率(26/30,86.7%)比教学组(14/30,46.7%)和自学组(12/30,40.0%)高,且3组通过率比较差异有统计学意义(χ2=10.8,P=0.001;χ2=14.067,P=0.0002;χ2=0.2715,P=0.6023)。结论规范和积极的STEP课程培训是超声医师介入培训合格和资格上岗的重要步骤和必要手段。Objective Ninety qualified physicians were chosen to take the training of STEP(screening,theory,examination,practice),in order to explore the training methods of interventional ultrasonic physicians.Methods According to different training methods,they were randomly divided into three groups:training-group,textbooks-group and self-study group.Each group experienced three study-stages which were theoretical study,simulation operation and practice operation,At the end of each stage,an evaluation was held and the eligible ones could enter the next stage.Results After the first two stages,the passing rate(28/28,100%)and average score(84.6±8.0)of training group were higher than the textbooks-group(16/21,76.2%,69.5±10.0) and self-study group(15/20,75.0%,67.3±13.0),and the passing rate of the three groups had statistically differences(χ2=5.0531,P=0.0246;χ2=5.3645,P=0.0206),while there were no significant differences between the last two groups(χ2=0.0757,P=0.7833);compared with the other two groups(14/16,87.5%;12/15,80.0%),the passing rate of training group afer the last stage(26/28,92.9%)had no statistically differences(χ2=0.0025,P=0.9605;χ2=0.5692,P=0.4506);after the study of three stages,the passing rate of training group(26/30,86.7%)was higher than that of the other two groups(14/30,46.7%;12/30,40.0%),and the differences had statistically significance(χ2=10.8,P=0.001;χ2=14.067,P=0.0002;χ2=0.2715,P=0.6023).Conclusion Thus normative and active STEP courses are important and necessary for interventional ultrasonic physicians to obtain qualifications.
