肌电图、神经电图在格林—巴利综合征中的应用

摘  要:目的:探讨肌电图(EMG)、神经电图在格林-巴利综合征(GBS)中的特点。方法:对31例GBS患者进行运动神经124条、感觉神经93条及128块肌肉进行EMG;运动神经传导速度(MCV);远端潜伏期;F波的出现率、潜伏期、波形;感觉神经传导速度(SCV)及一过性离散(TD)测定。结果:每例均有3条以上神经受累;远端潜伏期延长58%;MCV减慢70%;SCV减慢50%;F波异常率73%;TD异常55%。EMG出现自发电位占32%。结论:GBS为广泛的周围神经远端,近端损害,感觉运动均受累,存在以脱髓鞘为主伴有轴索变性的特点。EMG、神经电图的无创性,简便及可重复性使其成为GBS极为重要的诊断手段。Objectives: To study the characteristic of electromyogram (EMG) and electroneurogram of Guillain - Barre Syndrome (GBS). Methods: EMG and electroneurogram study were carried out on 124 motor nerves, 93 sensory nerves and 128 muscles in 31 cases of GBS. The parameters used included motor conduction velocity (MCV); distal latency; present rate, latency and abnormal forms of F wave, sensory conduction velocity (SCV) transient dispersion (TD). Results-.More than 3 nerves were involved in each case. In the 124 nerves examined, prolongation of the distal latency was found in 59 % > reduction of MCV in 70%, presence of F wave<50%. prolongation of latency of F wave in 73%; reduction of SCV in 50%, abnormal of TD in 55%. EMG appesrs autogenesis potential in 32%. Conclusion: GBS is a peripherial demyelination neuropathy involving both the proximal and distal segments. Both motor and sensory nerve were involved. The convenience, noninvasiveness, and repeated reproducibilities of the EMG and electroneurogram suggested that such techniques might provide important diagnostic criteria for GBS.
